Facility Inspections
While this nursing home's overall score was as bad as it gets, it performed well in the category of inspections. In fact, we awarded it a B for that area, which is one of our better scores. Perhaps the most critical factor we consider in calculating our inspection grades is deficiencies. Deficiencies are found on a facility's inspection reports. It is typically best to avoid facilities that have a long list of severe deficiencies. Although this place had a few deficiencies on its government inspection report, none of them were major deficiencies based on CMS' deficiency scale. A few minor deficiencies are not necessarily the end of the world.
Nurse Quality
This nursing home really excelled in the category of nursing, where it earned an D. Our nursing grade is primarily based on a nursing home's nurse staffing. This nursing home provides 0.4 hours of nursing care per resident per day. This is a much lower figure than we are used to seeing. Lastly, we also assessed some nursing quality measures in computing our nursing ratings. This place performed well when it comes to minimizing its residents' major falls and pressure ulcers. We consider these metrics to be reliable measures of the quality of nursing care.

Long-term Care Quality
Turning to the area of long-term care, this facility received just a D in this area. When facilities receive a grade in this range in long-term care it generally means the facility didn't perform well in our measures relating to patient care. After looking at the volume of care provided by nurses and other staff, we then looked at the nursing home's vaccination data. Candidly, we were a bit bothered by the fact that this nursing home vaccinated 84.210526 percent of its patients against pneumonia, which happens to be significantly less than average. To our surprise, this nursing home actually fared well at keeping its patients out of the hospital. In fact, it had just 0.03 hospitalizations per one thousand long-term resident days. This was its best feature in this category.
Short-term Care Quality
The next category we looked at was short-term care, where this nursing home was given a very poor grade here as well. We gave it an abysmal F in this category, which is obviously a major concern. In forming our short-term care ratings, we assess the facility's skilled nursing services, including those performed by registered nurses, speech therapists, physical therapists and other types of therapists. Our goal is to devise a barometer for comparing the rehabilitation services of different nursing homes. As you might expect, we discovered that this place offers significantly less physical therapist and registered nurse hours per resident than a typical facility. Lastly, we considered the percentage of patients who eventually returned home from this facility. This facility didn't fare well here either. In fact, we found that just 3.6 percent of this facility's patients were able to return home.
