Facility Inspections
Although we gave this facility a terrible overall grade, we awarded it a B- for our inspections rating. In fact, the facility received very impressive inspection report this year. Inspection grades are tied to several datapoints located in the a nursing home's recent inspection reports. Deficiencies are a key thing to look for on these reports. Severe deficiencies indicate a threat to the well being of residents. While this facility had some deficiencies on its government inspection report, none of them were serious based on CMS' deficiency scale. A couple minor deficiencies are not the end of the world.
Long-term Care Quality
We were also stunned to learn that this nursing home received a reasonably solid long-term care rating this year. We awarded it a grade of C in that area. For prospective residents looking for a permanent place to live rather than skilled nursing, long-term care grades are an important measure. One of the factors we considered after nursing hours was vaccines. Thankfully, this facility administered the pneumonia vaccine to 100 percent of its patients. Finally, this place was also able to limit hospitalizations. In fact, it had only 0 hospitalizations per one thousand long-term resident days, which is an impressively low figure.

Short-term Care Quality
This nursing home's second worse score is short-term care, where it earned just a D in that area. This is a well below average grade. Our short-term care grade is commonly employed to score a facility's performance with rehabilitation To offer quality rehabilitation services, nursing homes generally must have higher levels of skilled nursing. These types of services include registered nurses, physical therapists, occupational therapists and other highly skilled individuals. With its abysmal score in this category, we weren't shocked to learn that this facility offers significantly fewer registered nurse and physical therapist hours per resident than most facilities. Finally, we looked at the percentage of patients who were able to return to the community from this nursing home. This was not a strength for this facility. We found that just 0 percent of this facility's residents returned home. At most nursing homes, about half of their residents are able to return home so this is well below average.
Nurse Quality
Turning to our fourth area, we awarded this facility a grade of D for our nursing category. This is the facility's least impressive category rating. Our nursing score is largely based on the nursing home's level of nurse staffing. This place averaged a meager 0.3 hours of nursing care per resident each day. In addition to its weak nursing hour totals, this nursing home received suboptimal marks in many of the quality-based measures we considered in determining our nursing scores. We found that more of this facility's patients experienced falls which led to serious injury. We would argue that many falls could likely be prevented with better nursing care. This metric contributed to this facility receiving a poor nursing grade.
