This nursing home received a solid overall grade as described above, but it really excelled in nursing care, with an impressive grade of A in this category. Nursing ratings are tied to quantity of nursing care available. This nursing home averages 0.4 hours of nursing care per resident per day. Finally, our nursing grades also factor in quality-based assessments, such as avoiding major falls. This facility performed well in this area. Avoiding major falls is typically a good indicator that a facility has reliable quality controls in place. Major falls can many times be prevented if more nurses aids and better safety protocols are in place.

Long-term Care Quality

Grade: A

An additional strength of this nursing home is its top-shelf grade in long-term care. We awarded it one of our best grades in that area, with a score of A. For patients looking for a permanent place to live rather than rehabilitation, long-term care grades are an important measure. After considering the quantity of care provided by nurses and other staff, we next considered the facility's vaccination data. This facility vaccinated 88.38476 percent of its residents for pneumonia, which is a bit lower than we were hoping for but still a reasonably acceptable figure. Although its vaccination rate was a bit lower than we hoped for, we were pleased to find place was able to limit hospitalizations. In fact, it had only 0.02 hospitalizations per 1,000 long-term resident days, which is an impressively low number.

Short-term Care Quality

Grade: B-plus

In addition, we also wanted to draw your attention to the fact that this facility was awarded positive short-term care marks this year. We awarded them one of our more favorable grades in that area, with a B+. In the category of short-term care, we strive to evaluate measures of a facility's rehabilitation. We look at a nursing home's skilled nursing services, such as those performed by registered nurses and physical and occupational therapists. Fortunately, it appear that this nursing home has registered nurses on staff. Not every facility employs these types of nurses. On the other hand, according to the data this nursing home provided, it does not look like the facility employs physical therapists. Lastly, we assessed the percentage of residents who were able to return to the community from this nursing home. We found that 4.8 percent of this nursing home's residents returned home.

Facility Inspections

Moving on our final category of inspections, we gave this facility a D. This turned out to be this nursing home's worst grade and it really pulled its overall grade down. Our inspection score is based on the nursing home's government inspections. Since the data is collected by government inspectors, we weight it heavily in our assessment. One of the most important factors in assessing an inspection report is deficiencies. We would pay close attention to the level of severity of those deficiencies. This particular facility was hit with a category G through L deficiency, which are among the more serious categories of deficiencies. These categories indicate that the deficiencies identified by CMS had the potential to create actual harm to residents. Finally, we would like to draw your attention to the fact that this nursing home was assessed large government fines in recent years amounting to over $100,000. This generally indicates that a facility's poor track record spanned several years.
