Nurse Quality
To go along with its impressive overall score, we gave this nursing home a grade of A for our nursing score. Our nursing rating is primarily associated with the facility's nurse staffing. This facility provided 4 hours of nursing care per resident per day. This is an impressive figure which typically leads to higher quality care. Finally, this facility also performed well in several of the quality-based metrics we looked at. By way of illustration, in terms of the number of its patients sustaining falls leading to serious injury, this place performed as well as any facility in the state. Avoiding major falls is typically a good indicator that a nursing home has reliable quality controls in place. Major falls can frequently be avoided if more nurses aids and better safety protocols are in place.
Short-term Care Quality
Adding to this nursing home's list of achievements, it also was awarded a high-end short-term care grade. We gave them a dominant strong of A- in this category. In the category of short-term care, we try to qualify measures of a facility's rehabilitation services. We assess the nursing home's skilled nursing services, including the ones performed by registered nurses and various therapists. This nursing home elevated its grade in this category by offering more care from registered nurses to its residents than the average facility. The final measure we assessed in this category is the percentage of residents who eventually returned home from the facility. We found that it fared as well as just about any nursing home in Minnesota in this area with 58.5 percent of its patients returning home.

Long-term Care Quality
Turning to another strength for this nursing home, we awarded them one of our better scores in our long-term care category as well, with a grade of B. When nursing homes receive a grade in this range in long-term care it generally means it has plenty of staff and is an overall good place to reside on a permanent basis. One of the factors we considered after nursing hours is vaccines. This facility administered the pneumonia vaccine to 97.916664 percent of its residents. Vaccination is a proven method to avoid unnecessary hospitalizations for the nursing home population. The last statistic we assessed was its hospitalization rate. We found that this nursing home had 3.73 hospitalizations per one thousand long-term resident days. Although this wasn't as impressive as most of its other scores in this category, this figure can be skewed for some nursing homes due to the medical complexity of residents.
Facility Inspections
Moving on to the final category of inspections, this was not one of this nursing home's better grades. Inspection grades are based on a facility's recent government inspections. We awarded this facility a grade of C in this category. We believe that one of the most important factors in evaluating an inspection report is deficiencies. We especially focus on the severity of those deficiencies. This facility was assessed 8 deficiencies on its inspection report, but fortunately none were in categories G through L. This means CMS did not consider any of the deficiencies to create an imminent danger to resident safety or health. We did not consider anything on this nursing home's inspection reports to be overly concerning.
