South Davis Community Care Center is a small hospital-based facility in Bountiful, Utah. This nursing home turns out to be a grade A facility, which is a phenomenal grade. To go along with its strong overall score, we awarded this nursing home an A for our short-term care rating. Our short-term care scores are considered to be more meaningful for those in need of rehabilitation services. Rehabilitation typically utilizes additional highly-skilled nursing. This includes not only nursing services, but also physical and speech therapy, as well as other forms of therapy. When we assessed this facility's nursing hours, we found it offered substantially more care from registered nurses than most nursing homes. Lastly, we looked at the percentage of residents who were able to return home from this facility. We found that it outperformed the vast majority of facilities in the nation with 61.6 percent of its patients returning home. This is a substantially higher rate than most facilities.

Facility Inspections

Grade: A

Adding to this facility's impressive resume, we gave this nursing home an A for inspections. Our inspection scores account for a host of factors included in a facility's inspection report. One of the most important criteria we rely on is the quantity and severity of deficiencies. Facilities with better grades in this area generally have few of these severe deficiencies. While this nursing home had some deficiencies on its report, none were serious based on CMS' scale. Finally, this facility had no substantiated complaints this year from residents, which is an excellent sign.

Nurse Quality

Grade: A

This facility also excelled in the area of nursing, where it received a grade of A. Few nursing homes performed better in this area. There are a number of datapoints within this area. Many of these datapoints reflect nurse staffing. This particular facility provided 5 hours of nursing care per patient each day. This was one of the better totals we found. Furthermore, we found that a significant percentage of this care was provided by registered nurses, which are highly skilled nurses. On top of offering high levels of care, this place also performed well in several of the quality measures we looked at. In terms of the number of its patients suffering falls leading to major injury, this nursing home performed as well as any nursing home Utah.

Long-term Care Quality

Grade: B-plus

In the last category, this nursing home also was given an impressive long-term care score, with a grade of B+. This topped off a very strong report card. Facilities that excel in this category tend to be well-staffed and offer extensive hands on care to patients. In addition to providing very favorable levels of nurse staffing, this facility gave the pneumonia vaccine to 100 percent of its residents. Vaccines are vital to keeping residents out of the hospital. Finally, this nursing home was able to limit hospitalizations. It had less than one hospitalization per 1,000 long-term resident days, which is an impressively low figure.
