In fact, we awarded it an A+ for that category, which is one of our best scores. Our inspection grades weigh a host of factors found on a facility's inspection report. One key criteria we look at is the number and severity of deficiencies. You should avoid nursing homes with a list of deficiencies flagged. This facility received 4 deficiencies on its inspection report, but fortunately none of the deficiencies were considered to be severe. This indicates that the inspectors didn't deem any of these deficiencies to be an immediate risk to resident safety or health. A few minor deficiencies are not the end of the world.
Short-term Care Quality
We awarded this nursing home a grade of just D for our short-term care rating and that actually wound up as its second most favorable category. In the category of short-term care, we try to qualify indicators of a nursing home's rehabilitation. We look at a facility's skilled nursing services, such as the ones performed by registered nurses and various therapists. Considering its score in this area, we were not surprised to find that this nursing home was well below average in terms of its number of registered nurse and physical therapy hours provided to its residents based on the metrics we assessed. Frankly, this was about what we expected here. Lastly, we looked at the number of patients who returned home from this nursing home. This nursing home didn't fare well here either. In fact, we found that just 4.4 percent of this facility's residents were able to return home. This figure was quite a bit off the national average.

Long-term Care Quality
Sadly, this nursing home only received a grade of D for its long-term care rating, which is not a very good score. In a long-term care environment, the nursing home's primary objective is to keep patients as healthy and safe as possible. This is different than skilled nursing or short-term care where the goal is to rehabilitate patients. In addition to looking at the volume of care provided by nurses aids and other staff, we looked at the number of residents vaccinated for pneumonia. This facility vaccinated 98.23944 percent of its residents, which is better than most nursing homes. To our surprise, this facility was able to keep its patients out of the hospital. In fact, it had just 0 hospitalizations per 1,000 long-term resident days. Sadly, a few of its other scores in the category weren't as favorable as these.
Nurse Quality
Switching gears to the fourth area, this nursing home really did not fare well in this category either. Due to a very poor grade of F in nursing, this is about as as uninspiring as it gets. In computing a nursing home's nursing rating, we assess the amount of time nurses commit to residents as well as the level of licensure of those nurses.
