We are not able to award this place any respectable scores. We especially wanted to point out that it received an F for its inspection grade. An F in this area is really troublesome because it usually points to red flags on its government inspections. We would advise paying special attention to any deficiencies on a nursing home's inspection reports when it receives this poor of an inspection score. We suggest you scrutinize the level of severity of those deficiencies. This nursing home was hit with a category G through L deficiency, which are among the more serious categories of deficiencies. This generally indicates that the deficiencies uncovered by CMS had the potential to create actual harm to patients. Lastly, we want to draw your attention to the fact that this place received large government fines recently exceeding $50,000. This usually indicates that inspectors became frustrated after several years of failed inspections.

This facility wound up receiving a a terrible score in our long-term care area. It received a lowly F in this area. For prospective patients seeking a permanent residence as opposed to rehabilitation, long-term care is a very important category. Once we looked at the volume of care provided by nurses and other staff, we then analyzed the nursing home's vaccination records. This facility vaccinated 99.75903 percent of its residents against pneumonia, which is more than the vast majority of nursing homes. Surprisingly, this nursing home was able to limit hospitalizations. With only 0.8 hospitalizations per 1,000 long-term resident days, this facility has less hospitalizations than many nursing homes. Unfortunately, a few of its other scores in the category were not as favorable as these.

The next area we graded was short-term care. This facility's was given a low-end short-term care grade where it received a lowly F. In forming our short-term care grades, we analyze the nursing home's skilled nursing services, such as those performed by registered nurses, speech therapists, physical therapists and other types of skilled professionals. The purpose is to formulate a barometer for comparing the rehabilitation services of different facilities. Considering its score in this category, we were not stunned to find that this facility is well below average in terms of its quantity of physical therapy and registered nurse hours offered to its residents based on the data we looked at. Frankly, this was what we expected here. Lastly, we assessed the number of patients who were able to return to the community from this nursing home. We found that just 41.9 percent of this nursing home's patients returned home.

The last category we scored was nursing. Unfortunately, we gave it an F in this category, which is a rock bottom grade. We weighed the levels of nurses employed by the nursing home, in addition to the quantity of time those nurses worked with patients, in determining our rating in this category. With a meager 2.8 hours of nursing care per patient per day, this place's nurse staffing levels were quite a bit below average. To go along with its subpar totals in the area of nursing hours per resident, this place also did not fare as well in the quality-based measures we looked at in calculating our nursing scores. We looked at the percent of residents sustaining pressure ulcers and major falls. This facility had more falls and pressure ulcers than the average facility.
