Facility Inspections
We want to point out that this nursing home really outperformed its overall grade in the area of inspections. In fact, it received an excellent inspection report this year. As a result, we gave it one of our best grades in that area with a grade of A. Our inspection scores account for a host of factors included in a facility's inspection reports. One key criteria we rely on is the number and severity of deficiencies. Places with higher grades in this area generally have very few of these severe deficiencies. While this facility had some deficiencies on its government inspection report, none were major deficiencies based on CMS' deficiency scale. Finally, this facility had no substantiated complaints this year from residents, which is an excellent sign.
Long-term Care Quality
This facility also performed well in the category of long-term care, where it received a grade of A-. Not many nursing homes fared more favorably in this category. When nursing homes receive this type of score in this category it is usually a good sign for resident care and indicates that the place is well-staffed with nurses aids. On top of assessing the volume of care provided by nurses and other staff, we also looked at the number of residents vaccinated for pneumonia. This facility vaccinated 100 percent of its patients. Vaccines are critical to keeping patients healthy. Clearly, this place is doing something right in this area as it was able to keep its residents out of the hospital. Indeed, it had less than one hospitalization per 1,000 long-term resident days, which is a remarkably low figure.

Short-term Care Quality
Moving on to the area of short-term care, this nursing home was awarded a grade of just D in this category. Short-term care grades are generally employed to score a nursing home's rehabilitation services In order to have good rehabilitation services, nursing homes generally need to feature better levels of skilled nursing services. These types of services include registered nurses, physical therapists, occupational therapists and other licensed professionals. Unfortunately, we found that this nursing home offered fewer registered nurse and physical therapist hours per resident than most other facilities. The last metric we considered in this area is the percentage of residents who were able to leave the nursing home and return to the community. We discovered that just 4.5 percent of this facility's residents were able to return home, which is well below average. The combination of these poor statistics was damning for this facility's short-term care grade.
Nurse Quality
This facility's least impressive area is nursing, which is the final area we assessed. We gave it an F in this area. In spite of some decent grades in other areas, this score is still concerning so we wanted to make a note of it. Our nursing score weighs a host of factors. The most heavily weighted factor is the number of hours nurses spend with residents. This particular nursing home offered just 0.4 hours of nursing care per resident each day. This statistic is quite a bit below average. Finally, we also looked at some nursing quality measures in determining our nursing grades. This facility fared well when it comes to minimizing its residents' major falls and pressure ulcers.
