Facility Inspections
The primary reason this ended up being a decent nursing home is that it earned an elite inspection score. In fact, its inspection grade was far better than its overall grade. In the inspections category, we gave this facility an A+. Arguably the most significant factor we look at in calculating our inspection grades is deficiencies. Deficiencies are found on a facility's recent inspection reports. Nursing homes with better grades in this area most likely avoided the more severe deficiencies involving patient abuse or death. While this nursing home had some deficiencies on its report, none of them were severe based on CMS' scale. Finally, this facility had no substantiated complaints this year from residents, which is an excellent sign.
Long-term Care Quality
This nursing home also performed favorably in the category of long-term care, where it received a grade of A. Very few nursing homes fared more favorably in this category. Facilities that receive this type of score in long-term care typically provide consistent around the clock care to make sure patients are kept in good health. On top of considering the quantity of care provided by nurses and other staff, we also looked at the number of residents vaccinated for pneumonia. This nursing home provided the vaccine to 100 percent of its residents. Vaccines are critical to keeping patients healthy. Lastly, this place was able to limit hospitalizations. With just 1.35 hospitalizations per one thousand long-term resident days, this nursing home had less hospitalizations than many nursing homes.

Nurse Quality
Next, we looked at the nursing category where we gave this nursing home an F. This is certainly not one of this facility's better areas. Nursing ratings are based largely on quality and quantity of nursing care. This nursing home provides 3.2 hours of nursing care per resident per day. This is a much lower figure than we are used to seeing. In addition to its weak nursing hours, this place earned suboptimal marks in many of the quality-based measures we looked at in determining our nursing ratings. We found that a higher percentage of this nursing home's patients sustained falls which led to serious injury.
Short-term Care Quality
The final category we rated was short-term care, which turned out to be this nursing home's least impressive area. Unfortunately, we gave it an abysmal F for this area, which is clearly concerning. In our short-term care category, we endeavor to craft a meaningful measure for rehabilitation. In doing so, we assess the facility's level of highly-skilled nursing services, including both registered nurses and physical therapy, as well as respiratory therapy. It looks like this nursing home was subpar in terms of its quantity of registered nurse and physical therapy hours provided to its patients based on the metrics we assessed. Lastly, we looked at the percentage of residents who were able to return to the community from this nursing home. We found that just 34.9 percent of this facility's residents returned home.
