Facility Inspections
While this nursing home's overall grade was decent, it really excelled in the area of inspections. In fact, we awarded it a grade of A+ for that category, which is one of our best scores. Our inspection grades weigh a host of factors found on a facility's inspection report. One of the most important criteria we weigh heavily is the quantity and severity of deficiencies. Facilities with higher scores in this category typically have very few of these severe deficiencies. Although this facility had some deficiencies on its inspection report, none were serious based on CMS' deficiency scale. Finally, this facility had no substantiated complaints this year from residents, which is an excellent sign.
Long-term Care Quality
This facility also performed favorably in the area of long-term care, where it received a score of A-. Few nursing homes fared more favorably in this area. Facilities that receive this type of score in long-term care tend to provide consistent 24/7 care to make sure residents are well cared for. After we finished looking at the quantity of care provided by nurses, we turned to the facility's vaccination records. This facility vaccinated 95.833336 percent of its patients for pneumonia, which is an impressive figure. Pneumonia tragically can be a life threatening health condition for nursing home patients so we prefer when a facility does not roll the dice on this issue. Finally, this place was able to limit hospitalizations. In fact, it had only 0.02 hospitalizations per 1,000 long-term resident days, which is a remarkably low number.
Does this page answer your questions about Signature Healthcare of Monroe County Rehab & Wellness Center?
Short-term Care Quality
We gave this nursing home just a D for our short-term care grade. This is well below average in this category. Our short-term care scores are likely most important for people in need of rehabilitation services. Rehabilitation usually mandates more highly-skilled nursing. Skilled nursing means not only nursing, but also physical and occupational therapy, as well as other variations of therapy. Unfortunately, we found that this nursing home provided fewer physical therapist and registered nurse hours per resident than most other facilities. The last metric we assessed in this category is the percentage of patients who returned home from the nursing home. This nursing home struggled quite a bit in this area as well, with just 4.3 percent of its patients returning home. Unfortunately, this was quite a bit below average.
Nurse Quality
This nursing home's least impressive area is nursing, which is the last area we assessed. We gave it a lowly F for this area. Even with a few decent grades in other areas, this grade is still disappointing so we would like to to make sure you are aware. Nursing scores are based in large part on quality and quantity of nursing care. This facility provided just 0.4 hours of nursing care per resident per day. This is a very low total compared to most nursing homes. To go along with its subpar totals in the area of nursing hours per resident, this facility also did not fare as well in the quality-based measures we looked at in calculating our nursing scores. We looked at the percentage of residents sustaining major falls and pressure ulcers. This place had more falls and pressure ulcers than the average facility. This could be a bad sign when you consider that many bed sores and falls are believed to be preventable with better nursing care. Unfortunately, these metrics pulled down this facility's nursing grade quite a bit.
