Facility Inspections
Although this facility's overall grade was as bad as it gets, it actually fared pretty well in the area of inspections. In fact, we awarded it a B for that area, which is one of our better scores. Inspection ratings weigh several factors, including deficiencies, substantiated complaints and federal fines. You can learn more about each of these issues by reviewing copies of nursing homes' inspection reports. While this place had a few minor dings on its government inspection report, it had zero severe deficiencies. Severe deficiencies are ones found in categories G through L. This tells you that the inspectors did not consider any of the deficiencies on this facility's report to pose an imminent threat to patient health or safety. A couple minor deficiencies aren't necessarily the end of the world.
Short-term Care Quality
Unfortunately, this nursing home's next most favorable category was short-term care. However, with a D in this area, it still fared relatively badly. Short-term care grades are generally employed to assess a facility's performance with rehabilitation. To have quality rehabilitation services, nursing homes generally need to provide higher levels of skilled nursing. Skilled nursing includes registered nurses, physical therapists, speech therapists and other types of therapists. Considering its score in this area, we weren't stunned to discover that this place is well below average in terms of its quantity of registered nurse and physical therapy hours provided to its residents based on the data we looked at. The last measure we looked at in this area is the percentage of residents who ultimately were able to return home from the facility. This nursing home struggled quite a bit in this area as well, with just 0 percent of its patients returning home. Unfortunately, this was quite a bit below the national average.

Long-term Care Quality
Additionally, we gave this facility a rock-bottom F in the area of long-term care. For residents in need of a permanent place to live rather than skilled nursing, long-term care is a very important category. On top of considering the volume of care provided by nurses and other staff, we also looked at the number of residents vaccinated for pneumonia. Thankfully, this facility provided the vaccine to 98.32776 percent of its residents, which is a very solid percentage. To our surprise, this place was able to limit hospitalizations. With only 0.01 hospitalizations per 1,000 long-term resident days, this nursing home has less hospitalizations than the majority of nursing homes. Sadly, some of its other scores in the category were not as favorable as these.
Nurse Quality
The final area we scored is nursing. Unfortunately, we gave it an abysmal F for this category, which is a bottom of the barrel grade. There are quite a few criteria within this category. Many of the criteria are tied to staffing levels. This facility offered a meager 0.3 hours of nursing care per patient per day. To go along with its subpar totals in the area of nursing hours per resident, this facility also didn't fare as well in the quality-based measures we looked at in calculating our nursing grades. We looked at the percent of patients experiencing pressure ulcers and major falls. This place had more falls and pressure ulcers than the average facility.
