Facility Inspections
While we were not high on this place's overall performance, it actually received an impressive inspection report this year with few dings from the applicable government inspectors. As a result, it received one of our better grades in that area with a B+. Our inspection scores are tied to several items found on the a nursing home's recent inspection reports. Deficiencies are a key thing to look for on these reports. Most importantly, you should avoid facilities that have severe deficiencies associated with risks to resident well being. Fortunately, although this facility had a few minor dings on its inspection report, it had no severe deficiencies. Severe deficiencies are those labeled as categories G, H, I, J, K and L. This tells you the government inspectors didn't deem any of the deficiencies on this facility's report to pose an imminent threat to patient health or safety. A couple minor deficiencies aren't necessarily the end of the world.
Short-term Care Quality
The second best area we awarded this facility in any area came in the category of short-term care, where we gave it a grade of C. Our short-term care grades are likely more meaningful for folks needing rehabilitation services. Rehabilitation generally mandates additional highly-skilled nursing services. This includes not only nursing, but also physical and occupational therapy, as well as other types of therapy. Unfortunately, we found that this nursing home provided fewer registered nurse and physical therapist hours per resident than most other facilities. The final statistic we considered in this area is the percentage of residents who were able to leave the nursing home and return home. We discovered that just 0 percent of this facility's residents were able to return home, which is below average.

Long-term Care Quality
We also gave this nursing home a decent score in the category of long-term care, with a C. In forming these long-term care ratings, we assess the nursing home's personal care services. One of the criteria we considered in addition to nursing hours is vaccinations. Frankly, we were a bit bothered by the fact that this nursing home administered the pneumonia vaccination to just 89.52879 percent of its patients. On a more positive note, this nursing home was able to limit hospitalizations. Indeed, it had only 0.01 hospitalizations per 1,000 long-term resident days, which is a remarkably low figure.
Nurse Quality
The final category we scored was nursing, turned out to be this facility's worst category. We awarded this nursing home a grade of D in that area. Our nursing rating looks at the facility's nurse staffing levels. We look at both the levels of skill of those nurses as well as the quantity of hours spent with residents. This place provided just 0.5 hours of nursing care per resident per day. This is an alarmingly low total. This nursing home also had abysmal scores in some of the quality-based metrics to pair with its low nursing hours. We looked at the percent of residents experiencing pressure ulcers and we were very disappointed. This facility was at roughly 150 percent of the national average in this metric.
