Facility Inspections
While this facility's overall grade was not bad, it really excelled in the area of inspections. In fact, we awarded it a grade of A for that category, which is one of our best scores. Our inspection scores weigh several factors found on a facility's inspection report. One of the most important criteria we consider is the quantity and severity of deficiencies. Nursing homes with higher scores in this area tend to have very few severe deficiencies. This particular nursing home had just a single deficiency on its inspection report and it was not one deemed to pose a threat to patient health or safety. Based on this, we still had favorable view of this facility's inspections.
Long-term Care Quality
This nursing home was awarded its next best category grade in the area of long-term care. We gave this nursing home a B- in this area, which is an acceptable grade. In computing our long-term care ratings, we analyze the nursing home's personal care services. After considering the volume of care provided by aids and other staff, we turned to the nursing home's vaccination data. This nursing home vaccinated 100 percent of its residents for pneumonia, which is more than the majority of nursing homes. Lastly, this place was also able to limit hospitalizations. In fact, it had less than one hospitalization per 1,000 long-term resident days, which is an impressively low number.

Moving on to another strength for this facility, it also earned an elite nursing score. We gave this nursing home a grade of D. Nursing scores are mostly based on a nursing home's level of nurse staffing. With just 3 hours of nursing care per patient per day, this place's nurse staffing levels were far below average. To go along with its low total nursing hours per resident, this place had really poor marks in some of the quality-based measures we looked at in calculating our nursing grades. For starters, we looked at the percentage of patients having falls which led to major injury. This facility was at more than 150 percent of the national average in this statistic.
Short-term Care Quality
Moving on to our fourth area, this nursing home really didn't fare well. With a bottom of the barrel grade of F in short-term care, this was its worst grade. Our short-term care grade is commonly employed to grade a nursing home's performance with rehabilitation. To have highly rated rehabilitation services, nursing homes generally must have higher levels of skilled nursing services. These types of services include registered nurses, physical therapists, speech therapists and other licensed professionals. With its grade in this area, we weren't surprised to learn that this facility is well below average in terms of the quantity of physical therapy and registered nurse hours offered to its residents based on the data we assessed. The final measure we looked at in this category is the number of residents who were able to return home from the nursing home. This nursing home struggled quite a bit in this metric as well, with just 43.6 percent of its residents returning home. Unfortunately, this was well below the national average.
