Long-term Care Quality
While this place's overall grade was terrible, it actually didn't perform quite as poorly in long-term care. We awarded it a grade of C for that category, which isn't bad. This was not enough to salvage its otherwise weak profile. Long-term care grades focus on a facility's assistance with daily living instead of the highly skilled therapy and healthcare services that are critical to a rehabilitation facility. One of the factors we considered in addition to nurse's aid hours was vaccines. We were a bit alarmed this nursing home provided the pneumonia vaccine to a relatively low 52.35849 percent of its residents. On a more positive note, this facility was able to limit hospitalizations. Indeed, it had less than one hospitalization per 1,000 long-term resident days, which is an impressively low number.
Facility Inspections
As it turns out, its government inspection reports were not the source of this nursing home's rock bottom overall grade. It earned a C in this category. We take several aspects of a facility's inspection report into consideration in calculating our inspection grades. One critical factor is health deficiencies. It should be noted that the severity of the deficiencies is arguably more meaningful than the number of deficiencies, as some of these end up being relatively insignificant. Surprisingly, this facility was hit with a category L deficiency, which was not what we expected based on its inspection score. These are the most concerning deficiencies. This category indicates inspectors discovered a widespread deficiency placing resident health or safety in immediate jeopardy. Even a single serious deficiency of this nature should be taken seriously. In fact, it may even be worth doing more research into this deficiency and steps taken by the nursing home to prevent similar episodes in the future.

Nurse Quality
We gave this facility a grade of just D for our nursing rating. There are a variety of factors within this grade. Many of these factors are tied to the quantity of nurse staffing. This facility offered a meager 0.4 hours of nursing care per patient daily. This number is well below average. Finally, we also assessed some nursing quality measures in computing our nursing ratings. This facility fared well in the area of avoiding pressure ulcers and major falls. We consider these areas to be reliable indicators of the quality of nursing care.
Short-term Care Quality
Switching gears to our fourth category, this facility really did not perform very well in this category either. Due to a rock bottom grade of F in short-term care, this is about as as uninspiring as it gets. Our short-term care grade is typically used to judge a facility's performance with rehabilitation. To have highly scored rehabilitation services, facilities generally need to offer higher levels of skilled nursing. These types of services include registered nurses, physical therapists, occupational therapists and other highly trained professionals. With a score of this caliber in this category, we weren't shocked to discover this nursing home was below average in many staffing metrics we focus on. The nursing home supplied significantly fewer physical therapist and registered nurse hours per resident than most other nursing homes. The final datapoint we looked at in this area is the percentage of patients who were able to leave the nursing home and return to the community. We discovered that just 2.9 percent of this facility's patients were able to return home, which is well below average. The combination of these poor statistics sunk this facility's short-term care grade.
