Short-term Care Quality

Grade: B-minus

Even though we graded this facility incredibly poorly overall, it received a fairly respectable short-term care score this year. We gave it a B- in that area. In crafting these short-term care scores, we quantify the facility's skilled nursing services, such as those performed by registered nurses, speech therapists, physical therapists and other types of skilled professionals. Our goal is to formulate a barometer for sizing up the rehabilitation services of different facilities. This nursing home provides more services with physical therapists and registered nurses than most nursing homes we looked at. This is usually a favorable sign. Lastly, we considered the number of residents who were able to return to the community from this nursing home. We found that 5.2 percent of this facility's residents returned home. At most nursing homes, closer to half of their patients are able to return home so this figure is below average.

Nurse Quality

Grade: C

We were also surprised to find that this facility received a decent nursing grade this year. We gave it a grade of C in that category. Nursing scores are primarily associated with a facility's level of nurse staffing. This nursing home provides 0.3 hours of nursing care per resident per day. This is a much lower figure than we are used to seeing. Despite having low quantities of nursing care, this nursing home surprisingly fared better in some of the quality-based metrics we looked at. It performed well in terms of avoiding pressure ulcers and major falls. We consider these areas to be indicators of the quality of nursing care. In some cases, the quality of nursing care is as important as the number of hours of care provided.

Facility Inspections

Grade: F

We wanted to draw your attention to this nursing home's inspection rating where it received a lowly F. This often means we found red flags on its inspections. For nursing homes that received this bad of an inspection score, we recommend scrutinizing any severe deficiencies on its inspection report. This facility had one category G through L deficiency, which rank among the more serious categories of deficiencies. These categories indicate that the deficiencies found by inspectors had potential to pose actual harm to patients. Finally, we also need to draw your attention to the fact that this nursing home received large government fines in recent years amounting to more than $100,000. This generally signifies that a bad track record spanned several years.

Long-term Care Quality

Grade: F

The last area we looked at was long-term care, in which this nursing home received a rock bottom grade in this category as well. We gave it a lowly F for this category, which is obviously a major disappointment. When facilities receive this kind of grade in long-term care it is a bad sign for patient care and it may indicate that the nursing home is not as well-staffed with nurses and aids. Once we looked at the volume of care provided by nurses and other staff, we then looked at the facility's vaccination statistics. This nursing home vaccinated 92.73505 percent of its residents for pneumonia, which happens to be slightly below what we anticipated but still a decent percentage. To our surprise, this place was actually able to limit hospitalizations. With just 0.01 hospitalizations per one thousand long-term resident days, this facility had less hospitalizations than the average nursing home. This was its best score in this category.

Minimizes Serious Falls

Grade: F

In Sequim Health & Rehabilitation, 8.55% of Patients had Serious Falls

Minimizes Urinary Tract Infections

Grade: C

In Sequim Health & Rehabilitation, 2.96% of Patients had UTIs

This metric gauges the percentage of long-term patients that sustained a UTI. While more infections may reflect poorly on a facility's cleanliness, it is problematic to compare different nursing homes due to facilities having reporting standards.

Grade: B-minus

In Sequim Health & Rehabilitation, 15.93% of Patients use Anti-Psychotic Medication

This statistic tells you the percent of long-term residents who were prescribed antipsychotic medication. Excessive reliance on these medications may suggest a facility is using these drugs to control patient behavior in situations where such drugs aren't medically indicated. Nevertheless, some facilities need to rely more on these medications due to having more residents suffering from cognitive disorders.

Residents Maintain Autonomy

Grade: C

In Sequim Health & Rehabilitation, 18.88% of Patients

In Sequim Health & Rehabilitation, 19.6% of Residents Rehospitalized

This indicates the number of rehospitalizations per 1,000 days of short-term patient care.

Short-term Care: ER Visits

Grade: D

In Sequim Health & Rehabilitation, 17.28% of Patients

This tells you the number of emergency room visits per 1,000 days of short-term patient care. There is a correlation between keeping patients out of the emergency room and the overall quality of rehabilitation.

Short-term Care: Facilitates Functional Improvement

Grade: B

In Sequim Health & Rehabilitation, 77.26% of Resident

Measures the percentage of short-term stay patients that saw functional improvements, such as with activities of daily living. Many in the industry argue that this is a measure of a nursing home's rehabilitation performance.
