Long-term Care Quality

Grade: B-minus

Although we were not high on this facility overall, we awarded it a B- for long-term care. This is better than the nursing home's overall grade. Long-term care grades assess a nursing home's assistance with daily living instead of the highly skilled therapy and healthcare services that are critical to a skilled nursing facility. Once we finished looking at the quantity of care provided by nurses, we then analyzed the nursing home's vaccination statistics. Frankly, we were a bit bothered by the fact that this facility vaccinated only 90.645164 percent of its residents against pneumonia. Nevertheless, this place was able to keep its residents out of the hospital. In fact, it had only 0.01 hospitalizations per 1,000 long-term resident days, which is an impressively low number.

Facility Inspections

Grade: C

We awarded this nursing home a grade of C for our inspections category. Our inspection grades weigh several factors included in a facility's inspection report. One key criteria we look at is the quantity and severity of deficiencies. This facility was assessed 6 deficiencies on its government inspection report, but none were in category J, K or L. This means CMS did not deem any of the deficiencies to place patient health or safety in imminent harm or jeopardy. Unfortunately, we also need to draw your attention to the fact that this facility received significant government fines in recent years.

Nurse Quality

Grade: D

This facility's next best category was nursing, but even there it received a grade of just D. This is still a well below average grade. When computing our nursing ratings, we assess both nursing hours and the skill levels of the nurses. This facility provided just 0.3 hours of nursing care per resident per day. This is a very low total compared to most nursing homes. Finally, we also looked at some nursing quality-based metrics in calculating our nursing scores. This place performed well when it comes to avoiding pressure ulcers and major falls.

Short-term Care Quality

Grade: D

This facility's worst category was short-term care, which is the final area we assessed. For this category, we gave this nursing home a D. In computing these short-term care scores, we analyze the facility's skilled nursing services, such as those performed by registered nurses, occupational therapists, physical therapists and other types of skilled professionals. Our objective is to devise a tool for sizing up the rehabilitation services of nursing homes. With a grade this poor in this category, we weren't shocked to find this nursing home was below average in two of the key staffing areas we focus on. In fact, it supplied significantly fewer registered nurse and physical therapist hours per resident than most facilities. Finally, we assessed the percentage of residents who were able to return to the community from this facility. This was not a strength for this facility. We found that just 5.2 percent of this facility's residents returned home. At most facilities, around half of their patients are able to return home so this is well below average.
