Facility Inspections
Although we gave this facility a terrible overall grade, we awarded it a B- for our inspections rating. In fact, the facility received very impressive inspection report this year. We take several aspects of a facility's inspection report into consideration in determining these inspection scores. One critical factor is deficiencies. You should keep in mind that the severity of these deficiencies is arguably more meaningful than the number of deficiencies, as some end up being relatively minor. Although this place had some deficiencies on its government inspection report, none of them were serious based on CMS' deficiency scale. Lastly, this nursing home was cited by CMS for possible abuse or neglect. This is generally a really bad sign. We would never recommend this facility.
Short-term Care Quality
We were also stunned to discover that this nursing home earned a reasonably solid short-term care score this year. We gave it a B- in this area. In our short-term care grade, we seek to craft a meaningful barometer for rehabilitation services. In this process, we look at a nursing home's offerings of skilled nursing services, including both registered nurses and physical therapy, as well as speech therapy. We were surprised to find that this facility was below average in the two key staffing areas we assessed. It provided fewer physical therapist and registered nurse hours per resident than most other facilities. Lastly, we looked at the percentage of residents who were able to return to the community from this nursing home. We found that 5.4 percent of this facility's patients returned home. At most nursing homes, closer to half of their patients are able to return home so this figure is below average.

Nurse Quality
Turning to the area of nursing care, this facility received a grade of just D in this category. When determining a nursing home's nursing grade, we look at the amount of time nurses are caring for patients as well as the levels of training of those nurses. This place averaged only 0.3 hours of nursing care per resident each day. This is well below average. Finally, we also assessed some nursing quality-based metrics in calculating our nursing scores. This place fared well in terms of minimizing its residents' major falls and pressure ulcers.
Long-term Care Quality
The last area we assessed was long-term care, which proved to be a weak link for this facility. We gave this nursing home a grade of just D in long-term care. Nursing homes that receive this kind of score in this category may not provide the type of consistent around the clock care that some other nursing homes offer. After we finished assessing the amount of care provided by nurses, we then looked at the facility's vaccination data. We were a bit alarmed this facility vaccinated only 84.117645 percent of its patients for pneumonia. Surprisingly, this nursing home was actually able to limit hospitalizations. With only 0.03 hospitalizations per 1,000 long-term resident days, this facility has less hospitalizations than the average nursing home. This was its best score in this category.
