Note: Senior Care of Windcrest has been flagged for abuse in the past 12 months. We have capped this facility's overall grade accordingly.

Senior Care of Windcrest is a large nursing home located in San Antonio, Texas. This nursing home was given an F, which is definitely concerning in our grading scheme. We ranked this as one of the ten worst rated nursing homes in San Antonio. Given that the city offers countless other nursing homes, you should be able to find an alternative here. Finally, this facility had an ownership change in the last year, so hopefully its performance improves in the coming year.

Please note our records indicate that Senior Care of Windcrest has changed ownership recently and while our data on this page is up-to-date there is a chance that the quality of this facility may change under new management.

Even though we graded this nursing home incredibly poorly overall, it received a fairly respectable short-term care grade this year. We gave it a grade of B- in that category, which was the closest thing to a bright spot we can find for this nursing home. In computing our short-term care scores, we analyze a nursing home's levels of highly skilled nursing services, which includes registered nurses, physical therapists respiratory therapists and other highly skilled individuals. This category is often a meaningful measure of a nursing home's rehabilitation. In this place's case, we were surprised to learn that it actually provides less physical therapist and registered nurse hours per patient than the average facility. The hope is always that this does not adversely impact the quality of care. The last item we assessed in this category is the percentage of residents that ultimately were able to return home from the nursing home. This place fared much better in this metric with 60.5 percent of its residents returning home.

One of the other reasons we graded this facility so highly is that it received a very impressive nursing score. In that area, we gave this facility a grade of F. There are a number of criteria included in this area. Most of the subcategories relate to the quantity and quality of nurse staffing. This nursing home provides 3.3 hours of nursing care per resident per day. This is a much lower figure than we are used to seeing. Lastly, we also looked at some quality measures in calculating our nursing ratings. This facility was above average in two of the statistics we focus on, with solid marks for minimizing its residents' pressure ulcers and major falls.

We also wanted to draw your attention to this nursing home's inspection grades where it received an F. This generally means we found some red flags on the inspection reports. When a facility has this poor of an inspection score, you should look for severe deficiencies on its inspection report. Severe deficiencies indicate a threat to the well being of residents. This particular nursing home was assessed 4 deficiencies by CMS. The only positive thing we found is that none of the deficiencies were in the categories that suggest that they created a risk to patient safety or health. Finally, this facility was cited by CMS for possible abuse or neglect. We would not recommend this facility under any circumstance.

The next category we rated is long-term care. Unfortunately, we gave it an abysmal F for this category, which is a very poor grade. If you are looking for anything other than rehabilitation, you should take a close look at each facility's long-term care ratings. On top of considering the amount of care provided by aids and other staff, we looked at the number of residents vaccinated against pneumonia. This nursing home gave the vaccine to 98.20225 percent of its patients. To our surprise, this facility was able to limit hospitalizations. With just 0.8 hospitalizations per 1,000 long-term resident days, this facility has fewer hospitalizations than the majority of nursing homes. Sadly, some of its other scores in the category were not as favorable as these.
