Facility Inspections

Grade: D

On top of receiving a bad overall grade, we also gave this nursing home poor grades in the various categories. It only received just a D for its inspections score, which was its top area. One of the more critical factors in evaluating inspection reports is deficiencies. We recommend you pay close attention to the severity of the deficiencies. This particular nursing home was hit with 5 deficiencies on its inspection report. The only positive thing we can say is that none of the deficiencies were in the categories that indicate that they posed a threat to resident safety or health. Lastly, we need to draw your attention to the fact that this facility was assessed large government fines in recent years amounting to over $100,000. This often indicates that a bad performance spanned more than one year.

Nurse Quality

Grade: F

We also wanted to draw your attention to this facility's poor nursing grade where it received an F. The nursing rating weighs several components, many of which are tied to quantities of nurse staffing. This facility provided just 0.3 hours of nursing care per resident per day. This is a very low total compared to most nursing homes. Finally, we also assessed some nursing quality measures in calculating our nursing ratings. This nursing home performed well when it comes to minimizing its residents' pressure ulcers and major falls.

Long-term Care Quality

Grade: F

The next category we graded is long-term care. This facility's was given a low-end long-term care score where it was given a lowly F. Facilities that receive this type of score in long-term care may not provide the type of consistent 24/7 care that some other facilities provide. One of the factors we considered in addition to nursing hours is vaccinations. We were a bit alarmed this nursing home administered the pneumonia vaccine to only 30.57325 percent of its residents. Surprisingly, this facility was actually able to limit hospitalizations. With only 0.02 hospitalizations per one thousand long-term resident days, this facility had less hospitalizations than the majority of nursing homes. This is its best score in this category.

Short-term Care Quality

Grade: F

Moving on to the fourth area, this facility didn't fare very well in this category either. Due to a rock bottom grade of F in short-term care, this is about as terrible as it gets. In determining our short-term care scores, we scrutinize a facility's levels of skilled nursing services, which includes registered nurses, physical therapists occupational therapists and other licensed professionals. This category is considered to be a meaningful assessment of a nursing home's rehabilitation. As you might expect, we discovered that this facility offers substantially less registered nurse and physical therapist hours per patient than a typical nursing home. Finally, we looked at the percentage of patients who were able to return to the community from this nursing home. This wasn't a strength for this nursing home. We found that just 3.2 percent of this nursing home's patients returned home. At most facilities, about half of their patients are able to return home so this is well below average.
