Facility Inspections

Grade: C

Although we gave this facility a terrible overall grade, we awarded it a C for our inspections rating. In fact, the facility received very impressive inspection report this year. Inspection grades take several factors into consideration, including deficiencies and federal fines. You can learn more about each of these factors by reviewing copies of nursing homes' inspection reports. This nursing home was assessed 6 deficiencies on its government inspection report, but we were relieved to find that none were in any of the categories labeled J through L. This tells you inspectors didn't deem any of the deficiencies to put resident health or safety in imminent harm or jeopardy. Unfortunately, we also need to draw your attention to the fact that this facility received significant government fines in recent years.

Long-term Care Quality

Grade: F

This facility wound up receiving a an abysmal grade in our long-term care area. It was given an F in this area. Long-term care ratings of this caliber generally are a bad sign for the quantity and quality of care provided by nurses and aids, as well as the levels of routine medical care available. Once we assessed the quantity of care provided by nurses aids and other staff, we turned to the nursing home's vaccination data. Unfortunately, this nursing home vaccinated a relatively low 67.60564 percent of its patients against pneumonia. To our surprise, this nursing home was actually able to limit hospitalizations. With just 0.03 hospitalizations per 1,000 long-term resident days, this nursing home has less hospitalizations than many nursing homes. This was its best feature in this category.

Short-term Care Quality

Grade: F

Another concern was the F this facility received in the category of short-term care. In calculating our short-term care ratings, we assess the nursing home's levels of skilled nursing services, which includes registered nurses, physical therapists occupational therapists and other types of therapists. This category is generally a solid assessment of the nursing home's ability to rehabilitate patients. Not surprisingly, we found that this nursing home provides far less physical therapist and registered nurse hours per resident than the average facility. Finally, we considered the percentage of patients who were able to return to the community from this facility. We found that just 2.9 percent of this facility's patients returned home.

Nurse Quality

Grade: F

The last category we scored was nursing. It received a lowly F for this category, which is an abysmal grade. Nursing grades are based largely on nurse staffing levels. This place provided just 0.4 hours of nursing care per resident on a daily basis. This is well below average. On top of rating below average in nursing hours per resident, this place also had poor marks in the quality-based metrics we looked at in calculating our nursing ratings. We looked at the percent of residents sustaining pressure ulcers and major falls. This facility was at approximately 1.5 times the national average in both of these metrics. This is a bad sign when you consider that many bed sores and falls are believed to be preventable with better nursing care. These scores pulled down this nursing home's nursing rating quite a bit.
