Facility Inspections
While this facility's overall grade was as bad as it gets, it actually didn't perform terribly in the area of inspections. In fact, we awarded it a grade of B- for that category, which is one of our better scores. Our inspection scores account for several factors found on a nursing home's inspection report. One of the most important criteria we rely on is the quantity and severity of deficiencies. This place was assessed 4 deficiencies on its inspection report, but none were considered to be severe deficiencies. This tells you that the inspectors didn't deem any of the deficiencies to be an imminent risk to patient health or safety. A couple minor deficiencies are not necessarily the end of the world.
Long-term Care Quality
This facility also didn't fare poorly in the category of long-term care. In fact, we gave it a grade of C in that area, which happens to be another one of its better scores. Long-term care scores are pivotal for patients seeking non-healthcare services such as personal care. One of the criteria we considered after nursing hours is vaccinations. Thankfully, this nursing home provided the pneumonia vaccine to 100 percent of its residents. Lastly, this facility was also able to limit hospitalizations. In fact, it had less than one hospitalization per 1,000 long-term resident days, which is an impressively low number.

Nurse Quality
We were also surprised to find that this facility received a decent nursing grade this year. We gave it a grade of C in that category. When computing our nursing scores, we weigh both nursing hours and the levels of licensure of the nurses. This facility provided just 0.4 hours of nursing care per resident per day. This is a relatively low total compared to most other facilities. Lastly, nursing ratings also factor in quality-based measures, such as preventing major falls. Despite not having the most impressive total nursing hours per resident, this nursing home actually performed admirably in the metric of preventing major falls. This is usually a reliable indicator that a facility has quality controls in place. Some falls could be avoided if a facility provides enough nurses to assist its residents.
Short-term Care Quality
The final area we analyzed was short-term care, which ended up being this nursing home's least impressive area. We gave this nursing home a grade of D in this category. In our short-term care assessment, we try to forge a sound measure for rehabilitation services. In doing so, we analyze a nursing home's level of skilled nursing services, including both registered nurses and physical therapy, as well as occupational therapy. With a grade in this range for this category, we weren't stunned to discover this facility was below average in the two key staffing metrics we assess. In fact, it provided far fewer physical therapist and registered nurse hours per patient than most other nursing homes. The final item we looked at in this category is the percentage of patients that ultimately returned home from the nursing home. This facility struggled quite a bit in this area as well, with just 3.7 percent of its patients returning home.
