Facility Inspections
While we graded this facility incredibly poorly overall, it received a fairly respectable inspection report this year. As a result, it received a decent grade in that category with a score of B-. We take several aspects of a facility's inspection report into consideration in determining our inspection scores. One of those factors is health deficiencies. You should keep in mind that the severity of these deficiencies is usually more meaningful than the number of deficiencies, as some of these end up being relatively insignificant. Fortunately, although this facility had a few minor dings on its government inspection report, it had zero severe deficiencies. Severe deficiencies are ones found in categories G, H, I, J, K and L. This tells you CMS didn't consider any of the deficiencies on this facility's report to pose an imminent threat to patient health or safety. A few minor deficiencies aren't necessarily the end of the world.
Long-term Care Quality
This facility also didn't perform too badly in the area of long-term care. Indeed, we gave it a C for that area, which is among its more respectable scores. Our long-term care scores are more critical for individuals requiring more traditional assistance related to personal care. After we finished looking at the quantity of care provided by nurses, we turned to the facility's vaccination data. We were optimistic to discover that this nursing home vaccinates 99.62406 percent of its patients for pneumonia. Finally, this nursing home was also able to limit hospitalizations. In fact, it had less than one hospitalization per one thousand long-term resident days, which is an impressively low number.

Short-term Care Quality
Unfortunately, this facility's next best category was short-term care. However, with a D in this category, it still fared somewhat badly. With our short-term care grade, we seek to forge a meaningful gauge for rehabilitation services. In doing so, we assess a nursing home's level of highly-skilled nursing services, including both registered nurses and physical therapy, as well as occupational therapy. Considering its grade in this area, we were not stunned to learn that this nursing home was well below average in terms of the quantity of registered nurse and physical therapy hours offered to its patients based on the data we assessed. The final statistic we looked at in this area is the percentage of patients who were able to return home from the nursing home. This facility struggled quite a bit in this area as well, with just 4.9 percent of its patients returning home. Unfortunately, this was well below average.
Nurse Quality
The last category we looked at is nursing. This nursing home received a poor grade of F in that area. Our nursing rating is largely associated with the facility's level of nurse staffing. This place offered only 0.3 hours of nursing care per resident daily. This is not a very impressive figure. Lastly, we also looked at some quality-based metrics in computing our nursing grades. This nursing home was above average in two of the data points we considered, with solid marks for minimizing its residents' major falls and pressure ulcers. These data points are generally reliable measures of the quality of nursing care available.
