Facility Inspections
While this facility's overall grade was not a highlight, it actually received a somewhat respectable grade in the area of inspections. We awarded it a B- for this category, which is a decent score. We would argue that one of the most critical factors in assessing an inspection report is deficiencies. We especially look at the severity of the deficiencies. Amazingly, this was one of the few nursing homes in the country that had no deficiencies whatsoever on its inspection report. This is very impressive. Finally, this facility was cited by CMS for possible abuse or neglect. We would not recommend this facility under any circumstance.
Nurse Quality
This nursing home actually received a decent grade in the area of nursing as well. We awarded it a grade of B- for that category, which is not a bad score. Our nursing grade looks at the facility's level of nurse staffing. We factor in both the levels of training of those nurses as well as the number of hours spent with patients. This facility provided just 0.4 hours of nursing care per resident per day. This is a relatively low total compared to most other facilities. Finally, despite not having the highest total nursing hours per resident, this nursing home actually performed admirably in the metric of preventing major falls. This is usually a good indicator that a facility has sufficient quality controls in place.

Short-term Care Quality
This nursing home was awarded its next highest category score in the category of short-term care. We gave this nursing home a B- in this category, which is a respectable score. Our short-term care grades are important for prospective residents in need of rehabilitation. Rehabilitation usually mandates higher levels of skilled nursing. Skilled nursing includes a vast range of nursing services, ranging from registered nurses to physical and respiratory therapists, as well as other variations of therapy. Unfortunately, this facility was below average in terms of its quantity of physical therapy and registered nurse hours provided to its residents based on the measures we assessed. Finally, we looked at the percentage of patients who ultimately were able to return home from this nursing home. We found that 4.9 percent of this facility's patients were able to return home. This figure was actually well below average.
Long-term Care Quality
Our final category is long-term care, in which we gave this facility a grade of B- in this area. In our long-term care grade, we weigh the volume of care provided by a nursing home, along with a series of quality data points. One of the statistics we considered in addition to nurse's aid hours is vaccines. We were a bit concerned this nursing home administered the pneumonia vaccine to a relatively low 90.30837 percent of its patients. On a more positive note, this facility was able to limit hospitalizations. In fact, it had only 0.02 hospitalizations per one thousand long-term resident days, which is a remarkably low number.
