Sagecrest Alzheimers Care Center is a small non-profit nursing home located in San Angelo, Texas. This facility is ranked among the best nursing homes we found. In addition to being an elite nursing home overall, this nursing home also performed well in our inspections category, where it earned an A+. Few nursing homes fared this well in this area. Inspection scores are tied to items located in the a nursing home's recent inspection reports. Facilities that receive favorable grades in this area tend to have very few deficiencies on those reports. Most importantly, these nursing homes generally do not have any severe deficiencies which are associated with endangerment of patients. Fortunately, although this facility had a few minor dings on its report, it had zero severe deficiencies. Severe deficiencies are found in categories G, H, I, J, K and L. This means that the inspectors didn't deem any of the deficiencies on this facility's report to pose an imminent threat to patient health or safety. Finally, this facility had no substantiated complaints this year from residents, which is an excellent sign.

In addition, this facility received high marks in the area of nursing. As a result, it received one of our highest grades in that category with a score of A+. This is an extremely impressive statistic which typically leads to higher quality care. Lastly, this place also performed well in several of the quality-based metrics we assessed. For example, in terms of the percentage of its residents suffering falls leading to major injury, this nursing home performed as well as any nursing home in the nation. Avoiding major falls is typically a good indicator that a facility has reliable quality controls in place. Major falls can typically be avoided if more nurses aids and better safety protocols are in place.

This nursing home also was elite in the category of short-term care, where we gave it a score of A-. Few facilities performed better in this area. Our short-term care grades are based on a facility's quantity of skilled skilled healthcare professionals. This means a vast spectrum of services, ranging from registered nurses to physical and occupational therapists, not to mention other types of therapy. This nursing home excelled in the two key staffing areas we looked at. In fact, it supplied more care from registered nurses and physical therapists than the average nursing home. This is often an encouraging sign. The final datapoint we looked at in this area is the number of patients who were able to leave the nursing home and return to the community. We discovered that 41.2 percent of this facility's residents were able to return home.
Long-term Care Quality
In our last area, we gave this nursing home an A+ for our long-term care score. This rounded out a very favorable report card. Nursing homes that excel in this category typically are well-staffed and offer extensive hands on care to residents. In addition to considering the elite quantity of care provided by nurses aids and other staff at this facility, we also were impressed by the facility's vaccination data. By way of example, this facility administered the pneumonia vaccine to 100 percent of its patients, which is what we like to see. This combination proved to be successful as this facility also excelled at keeping its patients out of the hospital. It had only 0.29 hospitalizations per 1,000 long-term resident days, which is an impressively low figure.
