Facility Inspections
While we graded this facility incredibly poorly overall, it received a fairly respectable inspection report this year. As a result, it received a decent grade in that category with a score of B-. Our inspection grades account for several factors found on a nursing home's inspection report. One key criteria we weigh heavily is the quantity and severity of deficiencies. This facility was assessed 2 deficiencies on its inspection report, but none of the deficiencies were considered to be severe. This indicates that the inspectors did not deem any of these deficiencies to cause an immediate risk to patient health or safety. Lastly, this facility was flagged by CMS for possible abuse or neglect. We prefer to avoid facilities with this designation. Please do your best to find an alternative nursing home.
Long-term Care Quality
Surprisingly, we gave this facility a B- for its long-term care rating, which really isn't too bad of a score. With our long-term care score, we focus more on the volume of services offered by a nursing home, as well as some quality metrics. In addition to considering the quantity of care provided by aids and other staff, we also looked at the number of residents vaccinated against pneumonia. This facility vaccinated 100 percent of its residents, which is higher than most nursing homes. This nursing home also excelled at keeping its residents out of the hospital. Indeed, it had only 0.03 hospitalizations per 1,000 long-term resident days, which is a remarkably low figure.

Short-term Care Quality
Remarkably, we awarded this facility a C for our short-term care grade, which isn't a bad grade. Our short-term care grade is generally used to assess a facility's rehabilitation services To have high-end rehabilitation services, facilities generally need to have higher levels of highly skilled nursing. Skilled nursing includes registered nurses, physical therapists, respiratory therapists and other highly trained professionals. In this facility's case, we were surprised to learn that it actually provides less registered nurse and physical therapist hours per resident than the typical facility. Lastly, we considered the percentage of residents who ultimately were able to return home from this nursing home. We found that 5.3 percent of this facility's residents were able to return home. This figure was quite a bit below the national average.
Nurse Quality
The last area we looked at was nursing. It received an abysmal F for this category, which is a bottom of the barrel score. We scrutinized the qualifications of nurses at the nursing home, as well as the number of hours the nurses were with patients, in calculating our rating in this area. This place provided just 0.3 hours of nursing care per patient daily. This is a very low total compared to most nursing homes. On top of its weak nursing hour totals, this place earned poor scores in many of the quality-based measures we considered in computing our nursing grades. We found that more of this place's patients suffered falls leading to major injury. We would argue that many falls could likely be prevented with better nursing care. This metric contributed to this nursing home receiving a terrible nursing score.
