Although we gave this facility a terrible overall grade, we awarded it a C for our inspections rating. In fact, the facility received very impressive inspection report this year. Inspection scores weigh several factors found on a facility's inspection reports. One of the most important criteria we consider is the number and severity of deficiencies. This place had 7 deficiencies on its inspection report, but none were category G through L deficiencies. This means CMS didn't consider any of the deficiencies to be an imminent threat to patient safety or health. Unfortunately, we also need to draw your attention to the fact that this facility received significant government fines in recent years.

This nursing home also didn't rate too badly in the area of long-term care. In fact, we awarded it a grade of C for that category, which is another one of its more respectable grades. If you are seeking services other than rehabilitation, you should take a close look at long-term care scores. In addition to considering the amount of care provided by nurses and other staff, we analyzed the percentage of residents vaccinated for pneumonia. This facility provided the vaccine to 80.26316 percent of its residents. This is quite a bit less than average. Fortunately, this facility was able to limit hospitalizations. In fact, it had less than one hospitalization per 1,000 long-term resident days, which is an impressively low figure.

We also wanted to draw your attention to this facility's poor nursing grade where it received an F. Our nursing rating assesses quite a few datapoints, but the main consideration is the number of nurse hours spent with patients. This place offered just 0.3 hours of nursing care per resident each day. This is well below average. Lastly, we also looked at some nursing quality measures in calculating our nursing scores. This nursing home fared well when it comes to minimizing its residents' pressure ulcers and major falls. We consider these datapoints to be good measures of the quality of nursing care.

The last area we assessed is short-term care. Unfortunately, we gave it an abysmal F for this category, which is an abysmal score. In the category of short-term care, we strive to assess measures of a nursing home's rehabilitation. We look at a facility's skilled nursing services, such as those performed by registered nurses and physical and occupational therapists. Not surprisingly, we found that this nursing home provides far less physical therapist and registered nurse hours per resident than the average facility. Finally, we assessed the percentage of residents who were able to return to the community from this facility. This definitely wasn't a strength for this nursing home. We found that just 4 percent of this nursing home's patients returned home. At most facilities, around half of their patients are able to return home so this is well below average.
