Facility Inspections
This nursing home earned a solid overall grade as described above, but we really wanted to emphasize its nearly flawless health inspection reports in recent years. We gave them an A in this category. We take several aspects of a facility's inspection report into consideration in calculating these inspection scores. One of those factors is health deficiencies. It should be noted that the severity of these deficiencies is arguably more meaningful than the quantity of deficiencies, as some deficiencies can be relatively minor. Although this facility had a few deficiencies on its report, none were major deficiencies based on CMS' deficiency scale. We should point out that deficiency-free inspections are rare in this industry.
Long-term Care Quality
This facility also excelled in the category of long-term care, where it received a grade of A-. Not many facilities fared better in this category. Facilities that do well in this category tend to provide patients with better supervision and stay on top of routine healthcare services. In addition to considering the quantity of care provided by nurses and other staff, we looked at the percentage of residents vaccinated against pneumonia. This nursing home vaccinated 98.80137 percent of its residents. Vaccines are vital to keeping residents out of the hospital. Clearly, this place is doing something right in this area as it also excelled at keeping its patients out of the hospital. Indeed, it had less than one hospitalization per 1,000 long-term resident days, which is a remarkably low figure.

Nurse Quality
Moving on to another strength for this facility, it also received an elite nursing grade. In fact, we gave this nursing home a grade of D. The nursing category analyzes many factors, many of which are associated with quantities of nurse staffing. This nursing home averaged just 0.4 hours of nursing care per resident each day. Lastly, we also assessed some nursing quality measures in calculating our nursing scores. This facility fared well when it comes to avoiding pressure ulcers and major falls.
Short-term Care Quality
The next area we graded is short-term care. It received a lowly F for this area, which is a very poor score. This is obviously a significant concern. In determining our short-term care scores, we scrutinize a facility's levels of skilled nursing services, which includes registered nurses, physical therapists speech therapists and other highly skilled individuals. This category is typically a meaningful measure of the facility's rehabilitation services. Unfortunately, this nursing home was below average in terms of its quantity of physical therapy and registered nurse hours provided to its residents based on the measures we assessed. Finally, we considered the number of residents who returned home from this facility. This nursing home didn't fare well here either. In fact, we found that just 3.5 percent of this facility's patients were able to return home.
