Facility Inspections
Although we gave this facility a terrible overall grade, we awarded it a B- for our inspections rating. In fact, the facility received very impressive inspection report this year. We believe that one of the most important factors in evaluating an inspection report is deficiencies. In this regard, we especially look at the severity of the deficiencies. While this place had some minor dings on its government inspection report, it had no severe deficiencies. Severe deficiencies are ones labeled as categories G, H, I, J, K and L. This means that the government inspectors didn't consider any of the deficiencies on this facility's report to pose an imminent threat to patient health or safety. Finally, this facility was cited by CMS for possible abuse or neglect. We would not recommend this facility under any circumstance.
Long-term Care Quality
This nursing home also didn't fare terribly in the area of long-term care. Indeed, we awarded it a grade of C in that area, which turned out to be among its better grades. In a long-term care setting, the primary objective is to keep patients healthy and safe. This is different than skilled nursing or short-term care where the goal is to rehabilitate patients. On top of considering the volume of care provided by nurses and other staff, we looked at the percentage of residents vaccinated for pneumonia. This facility vaccinated 97.945206 percent of its patients, which is better than most nursing homes. This nursing home also excelled at keeping its patients out of the hospital. Indeed, it had only 0.02 hospitalizations per 1,000 long-term resident days, which is an impressively low number.

Short-term Care Quality
Moving on to the category of short-term care, this facility was given a grade of just D in this area. Our short-term care grades are presumably most important for those in need of rehabilitation services. Rehabilitation typically mandates more skilled nursing. Skilled nursing includes not only nursing, but also physical and speech therapy, as well as other variations of therapy. Given its abysmal grade in this area, we were not surprised to find this nursing home provides far fewer physical therapist and registered nurse hours per resident than most nursing homes. The final measure we assessed in this area is the percentage of patients that eventually returned home from the facility. This nursing home struggled quite a bit in this metric as well, with just 5.1 percent of its residents returning home.
Nurse Quality
The last category we looked at was nursing, which is this nursing home's worst category. We awarded this facility a grade of D in this category. Our nursing grade analyzes the nursing home's nurse staffing levels. We weigh both the levels of training of those nurses and the number of hours spent with patients. This facility averaged just 0.3 hours of nursing care per patient daily. This number is well below average. Finally, we also assessed some nursing quality measures in determining our nursing scores. This nursing home performed well in the area of avoiding major falls and pressure ulcers.
