Facility Inspections
Although we gave this nursing home a terrible overall score, we awarded it an A- for our inspections score. This grade is far more impressive than the nursing home's overall grade. We weigh several aspects of a nursing home's inspection report in calculating our inspection scores. One of those factors is health deficiencies. It should be noted that the severity of the deficiencies is usually more meaningful than the quantity of deficiencies, as some deficiencies are relatively minor. Although this facility had a few deficiencies on its government inspection report, none of them were severe based on CMS' deficiency scale. A couple minor deficiencies aren't necessarily the end of the world.
Nurse Quality
Unfortunately, this facility only received a grade of D for its nursing grade, which was actually one of its better category grades. There are a host of subcategories included in this grade. Most of these criteria are tied to nurse staffing. This facility offered a meager 0.3 hours of nursing care per patient on a daily basis. This is well below average. Finally, we also assessed some nursing quality measures in calculating our nursing grades. This place performed well in the area of minimizing its residents' major falls and pressure ulcers.

Long-term Care Quality
Another unfortunate development is the F this nursing home received in the area of long-term care. In a long-term care setting, the nursing home's primary objective is to keep patients healthy and safe. This is different than skilled nursing or short-term care where the goal is to rehabilitate patients. In addition to considering the volume of care provided by aids and other staff, we also looked at the number of residents vaccinated against pneumonia. Fortunately, this nursing home vaccinated 96.526054 percent of its residents, which is a very respectable percentage. Surprisingly, this facility was able to keep its patients out of the hospital. It had only 0 hospitalizations per one thousand long-term resident days. Sadly, some of its other scores in the category were not as favorable as these.
Short-term Care Quality
The next category we rated was short-term care. This facility was given an abysmal grade of F in this category. With our short-term care assessment, we attempt to forge a meaningful barometer for rehabilitation services. In doing so, we look at the facility's level of skilled nursing services, including both registered nurses and physical therapy, as well as respiratory therapy. Given its poor score in this area, we weren't stunned to discover this facility provided far fewer physical therapist and registered nurse hours per resident than most other nursing homes. Finally, we assessed the percentage of patients who were able to return to the community from this facility. This was not a strength for this nursing home. We found that just 2.5 percent of this nursing home's patients returned home. At most facilities, about half of their residents are able to return home so this is well below average.
