Although we gave this place a terrible overall score, we gave it a C for our long-term care rating. Our long-term care grades are more important for individuals in need of more conventional assistance related to activities of daily living. On top of looking at the quantity of care provided by aids and other staff, we looked at the number of residents vaccinated for pneumonia. This facility vaccinated 100 percent of its residents, which is more than the majority of nursing homes. The last statistic we assessed was its hospitalization rate. We found that this nursing home had 3.66 hospitalizations per 1,000 long-term resident days. Although this figure is somewhat concerning, this statistic can be skewed for some nursing homes based on the medical complexity of residents.

Interestingly, this nursing home's awful overall score can't really be attributed to its government inspection reports. We awarded it a C for our inspections category. Our inspection grades weigh several factors found on a nursing home's inspection report. One key criteria we look at is the quantity and severity of deficiencies. To our surprise, this place was assessed a category J through K deficiency, which are among the worst possible categories of deficiencies. This generally means that inspectors uncovered a deficiency which places patient safety or health in imminent jeopardy. Finally, they also received 17 substantiated complaints in recent years. This is certainly not a great sign.

Unfortunately, this facility only received a grade of D for its nursing grade, which was actually one of its better category grades. Our nursing score is largely based on a facility's level of nurse staffing. Quantity of nursing care is not the issue here with this place's grade. This place actually provides an above average 3.9 hours of nursing care per patient on a daily basis. Lastly, we also looked at some quality measures in calculating our nursing grades. This nursing home was above average in two of the major areas we look at, with solid scores for minimizing its residents' pressure ulcers and major falls.

Switching gears to the final area, we gave this facility a D in short-term care. This was the facility's least impressive category grade. In determining these short-term care grades, we analyze the nursing home's skilled nursing services, such as those performed by registered nurses, occupational therapists, physical therapists and other types of skilled professionals. The objective is to create a measure for sizing up the rehabilitation services of various facilities. We found that this place was below average in the two key staffing areas we assessed. It offered fewer physical therapist and registered nurse hours per patient than most nursing homes. Finally, we looked at the percentage of patients who ultimately returned home from this facility. This facility didn't fare well here either. In fact, we found that just 37.6 percent of this facility's patients were able to return home. This figure was quite a bit off the national average.
