While we were not high on this facility's overall score, it actually received a very impressive inspection report this year. In fact, it received an excellent inspection report this year. As a result, it received one of our best scores in that category with an A. Our inspection scores are based on pieces of information located in the a nursing home's recent inspection reports. Deficiencies are a key thing to look for on these inspections. You should especially avoid nursing homes that have severe deficiencies linked to risks to resident well being. Fortunately, although this facility had a few minor dings on its inspection report, it had zero severe deficiencies. Severe deficiencies are those found in categories G, H, I, J, K and L. This tells you the inspectors didn't consider any of the deficiencies on this facility's report to pose an imminent threat to patient safety or health. A couple minor deficiencies aren't the end of the world.

One of this facility's higher category scores came in the category of long-term care. This proved to be its second most respectable category. In that area, we awarded this nursing home a grade of B-. For prospective residents looking for a permanent residence as opposed to skilled nursing, long-term care grades are a key measure. After considering the amount of care provided by nurses and other staff, we then looked at the nursing home's vaccination data. Frankly, we were a bit concerned this nursing home vaccinated 83.6299 percent of its residents for pneumonia, which is quite a bit below what we expected. Nevertheless, this place keeps its residents out of the hospital. In fact, it had only 0.02 hospitalizations per 1,000 long-term resident days, which is an impressively low figure.

In addition, we gave this facility an F in the area of nursing care. Our nursing grade is primarily associated with a facility's level of nurse staffing. With only 0.3 hours of nursing care per patient per day, this facility's nurse staffing levels were far below average. Finally, we also looked at some nursing quality-based metrics in computing our nursing grades. This facility fared well when it comes to avoiding pressure ulcers and major falls.

The final category we looked at was short-term care. This facility was given a very poor grade of F in that category. In calculating these short-term care scores, we assess the nursing home's skilled nursing services, including those performed by registered nurses, respiratory therapists, physical therapists and other types of therapists. Our purpose is to devise a tool for comparing the rehabilitation services of various facilities. With its score in this category, we were not stunned to discover that this nursing home is well below average in terms of its number of physical therapy and registered nurse hours offered to its patients based on the figures we looked at. Frankly, this was about what we expected to find. Finally, we assessed the number of residents who were able to return to the community from this facility. This definitely was not a strength for this facility. We found that just 4.3 percent of this facility's patients returned home. At most nursing homes, around half of their residents are able to return home so this is well below average.
