Facility Inspections
Although we gave this facility a terrible overall grade, we awarded it a C for our inspections rating. In fact, the facility received very impressive inspection report this year. These inspection grades take several factors into consideration, including deficiencies and substantiated complaints. You can learn more about each of these issues by reviewing copies of nursing homes' inspection reports. This nursing home was assessed 6 deficiencies on its inspection report, but we were relieved to find that none were category G through L deficiencies. This means inspectors did not consider any of the deficiencies to present an imminent threat to patient health or safety. Finally, this facility was cited by CMS for possible abuse or neglect. We would not recommend this facility under any circumstance.
Nurse Quality
We gave this facility a grade of just D for our nursing rating and that actually turned out to be its second highest score. Our nursing grade is primarily associated with a nursing home's nurse staffing. This nursing home offered a meager 0.4 hours of nursing care per patient per day. This is well below average. Finally, we also assessed some nursing quality-based metrics in determining our nursing scores. This facility performed well when it comes to avoiding major falls and pressure ulcers. We consider these datapoints to be indicators of the quality of nursing care.

Short-term Care Quality
Our next category we assessed is short-term care, in which this facility was given an F. Our short-term care scores are crucial for individuals in need of rehabilitation. Rehabilitation typically requires higher levels of skilled nursing services. Skilled nursing means a wide range of nursing services, ranging from registered nurses to physical and respiratory therapists, as well as other forms of therapy. With a grade in this range for this category, we were not surprised to find that this place was well below average in several staffing figures we looked at. It provided significantly fewer physical therapist and registered nurse hours per patient than most nursing homes. The last metric we considered in this area is the percentage of residents that were able to return home from the facility. This facility struggled quite a bit in this area as well, with just 3.7 percent of its patients returning home.
Long-term Care Quality
Moving on to the final area, this nursing home did not fare well in this area either. Due to a very poor grade of F in long-term care, this is about as terrible as it gets. When nursing homes receive a score in this range in long-term care it typically means the facility didn't perform well in our measures relating to patient care. In addition to assessing the amount of care provided by nurses aids and other staff, we also looked at the percentage of residents vaccinated for pneumonia. Candidly, we were a bit alarmed this facility administered the vaccine to a relatively low 90.18405 percent of its patients. To our surprise, this place was actually decent at keeping its patients out of the hospital. Indeed, it had just 0.01 hospitalizations per 1,000 long-term resident days. This is its best score in this category.
