Facility Inspections
While this place's overall score was as bad as it gets, it actually performed well in the area of inspections. In fact, we awarded it a B for that area, which is one of our better scores. Our inspection grades weigh several factors included in a facility's inspection report. One key criteria we consider is the number and severity of deficiencies. You should avoid places with a list of severe deficiencies flagged. Amazingly, this was one of the few nursing homes in the country that had no deficiencies whatsoever on its inspection report. This is very impressive. The lack of deficiencies proved to be this nursing home's best achievement.
Long-term Care Quality
Unfortunately, this nursing home's second best score was long-term care. However, with a D in this area, it still performed relatively badly. Facilities that do not fare well in long-term care typically are not as well-staffed and are lagging in some of the areas of routine healthcare services we looked at. After considering the volume of care provided by nurses and other staff, we next considered the facility's vaccination statistics. This nursing home vaccinated 92.51337 percent of its residents for pneumonia, which happens to be a bit less than we anticipated but still a respectable percentage. Surprisingly, this place was actually able to limit hospitalizations. With just 0.02 hospitalizations per 1,000 long-term resident days, this facility has less hospitalizations than the majority of nursing homes. This was its best feature in this category.

Short-term Care Quality
Sadly, this nursing home's third highest category was short-term care. However, with a grade of D in this category, it still fared somewhat poorly. In computing our short-term care ratings, we look at the nursing home's skilled nursing services, including those performed by registered nurses, occupational therapists, physical therapists and other types of skilled professionals. The goal is to formulate a measure for sizing up the rehabilitation services of various facilities. Not surprisingly, we found that this nursing home provides far less physical therapist and registered nurse hours per resident than the average facility. Lastly, we assessed the number of residents who were able to return to the community from this nursing home. We found that just 4.9 percent of this facility's patients returned home.
Nurse Quality
The final category we looked at was nursing. This facility received a rock bottom grade of F in that category. We scrutinized the levels of nurses at the facility, as well as the number of hours the nurses worked with residents, in computing our score in this area. This facility provided just 0.3 hours of nursing care per resident per day. This is a very low total compared to most nursing homes. To pair with its low totals in the area of nursing hours per patient, this nursing home also did not fare as well in the quality-based measures we looked at in computing our nursing ratings. We looked at the percent of residents sustaining pressure ulcers and major falls. This place had more pressure ulcers and falls than the average facility.
