Facility Inspections
Although we didn't grade this nursing home favorably overall, we actually gave it an excellent score in the area of inspections, where it received an A+. Inspection scores are tied to datapoints located in the a nursing home's recent inspection reports. Deficiencies are a key thing to look for on these reports. Most importantly, you should avoid facilities with severe deficiencies linked to endangerment of patients. This place received 3 deficiencies on its inspection report, but we were relieved to see that none of the deficiencies were considered severe. This means that the inspectors did not consider any of these deficiencies to pose an immediate risk to resident safety or health. A couple minor deficiencies are not necessarily the end of the world.
Short-term Care Quality
As it turns out, this facility only received a D for its short-term care score, which was actually one of its better category grades. Our short-term care grades are considered to be more critical for residents needing rehabilitation from their nursing home. Rehabilitation typically requires more skilled nursing services. This includes not only nursing, but also physical and speech therapy, as well as other forms of therapy. Not surprisingly, we learned that this nursing home provides significantly less registered nurse and physical therapist hours per patient than the typical facility. Finally, we assessed the number of residents who were able to return to the community from this facility. This definitely wasn't a strength for this nursing home. We found that just 0 percent of this nursing home's residents returned home. At most facilities, about half of their residents are able to return home so this is well below average.

Long-term Care Quality
Sadly, this facility received an F for its long-term care grade. If you are looking for anything other than rehabilitation, you should take a close look at each facility's long-term care ratings. In addition to considering the amount of care provided by nurses aids and other staff, we also looked at the number of residents vaccinated for pneumonia. This nursing home provided the vaccine to 100 percent of its patients. Surprisingly, this place was able to limit hospitalizations. With only 0.01 hospitalizations per 1,000 long-term resident days, this facility had fewer hospitalizations than most nursing homes. Unfortunately, some of its other scores in this category weren't as favorable as these.
Nurse Quality
The next area we looked at was nursing. This facility was given a bottom of the barrel grade of F in that category. We looked at the skill-level of nurses employed by the facility, as well as the number of hours the nurses spent with patients, in computing our rating in this area. This place averaged a meager 0.3 hours of nursing care per resident per day. This is a very low total compared to most nursing homes. In addition to ranking below average in nursing hours per resident, this place also had poor scores in the quality-based metrics we looked at in calculating our nursing grades. We looked at the percentage of patients experiencing major falls and pressure ulcers. This facility was at approximately 150 percent of the national average in both of these data points. This is likely a bad sign when you consider that many falls and bed sores are believed to be preventable with better nursing care. These metrics helped pull down this nursing home's nursing grade significantly.
