Although we gave this facility a terrible overall grade, we awarded it an A- for our inspections score. This grade is far more impressive than the facility's overall score. Inspection grades weigh a host of factors included in a nursing home's inspection reports. One of the most important criteria we look at is the number and severity of deficiencies. You should avoid places with too many severe deficiencies flagged. While this nursing home had some deficiencies on its government inspection report, none were serious based on CMS' deficiency scale. A couple minor deficiencies aren't necessarily the end of the world.

This nursing home really excelled in our nursing category, where it received an D. Nursing grades are primarily based on a nursing home's nurse staffing. This place provided a meager 2.6 hours of nursing care per patient daily. In addition to receiving below average scores for total nursing hours, this nursing home was a bit less impressive in some of the quality-based measures we looked at in computing our nursing grades. We looked at the percentage of patients suffering pressure ulcers and found that this place was at roughly 150 percent of the national average in this statistic.

The next category we graded was long-term care. This nursing home's was given a low-end long-term care score where it was given an F. Long-term care scores in this range generally are a bad sign for the quantity and quality of care provided by nurses and aids. One of the factors we considered in addition to nurse's aid hours was vaccinations. This facility provided the pneumonia vaccination to 90.90909 percent of its patients. To our surprise, this facility actually fared well at keeping its residents out of the hospital. In fact, it had only 0 hospitalizations per one thousand long-term resident days. This was its best feature in this category.

The final area we assessed is short-term care. This facility was given a very poor grade of F in that category. In computing our short-term care scores, we analyze the nursing home's skilled nursing services, including those performed by registered nurses, speech therapists, physical therapists and other types of skilled professionals. The purpose is to formulate a scale for comparing the rehabilitation services of various facilities. With a score of this caliber for this category, we were not stunned to find that this nursing home was well below average in many staffing areas we focus on. In fact, it provided substantially fewer physical therapist and registered nurse hours per patient than most other nursing homes. Lastly, we assessed the percentage of patients who were able to return home from this facility. This facility didn't fare well here either. In fact, we found that just 20.5 percent of this facility's residents were able to return home. This figure was quite a bit off the national average.
