Facility Inspections
Although this place's overall grade was as bad as it gets, it fared well in the category of inspections. In fact, we awarded it a B for that area, which is one of our better scores. We take several aspects of a facility's inspection report into consideration in computing our inspection grades. One key factor is deficiencies. It should be noted that the severity of these deficiencies is usually more important than the quantity of deficiencies, as some deficiencies end up being relatively minor. While this facility had a few deficiencies on its government inspection report, none of them were major deficiencies based on CMS' scale. A few minor deficiencies aren't the end of the world.
Short-term Care Quality
Moving on to the category of short-term care, this facility received a grade of just D in that category. Unfortunately, this ended up being one of its better features. Our short-term care grade is often employed to gauge a facility's performance with rehabilitation. To have highly rated rehabilitation services, nursing homes generally need to offer higher levels of skilled nursing. These types of services include registered nurses, physical therapists, occupational therapists and other highly trained professionals. With its grade in this area, we were not surprised to learn that this nursing home was well below average in terms of the quantity of physical therapy and registered nurse hours offered to its residents based on the figures we assessed. Honestly, this was about what we expected to find. The last metric we considered in this area is the percentage of residents who eventually returned home from the facility. This facility struggled quite a bit in this metric as well, with just 0 percent of its patients returning home. Unfortunately, this was well below average.

Long-term Care Quality
Another issue was the F this nursing home received in the area of long-term care. For patients in need of a permanent place to live as opposed to rehabilitation, long-term care is a very important category. On top of considering the volume of care provided by nurses and other staff, we also looked at the percentage of residents vaccinated against pneumonia. Thankfully, this nursing home gave the vaccine to 97.594505 percent of its patients, which is a very respectable statistic. Surprisingly, this nursing home was able to keep its patients out of the hospital. In fact, it had just 0.02 hospitalizations per 1,000 long-term resident days. Unfortunately, some of its other scores in this category were not as favorable as these.
Nurse Quality
Moving on to the last area, this nursing home really did not fare well in this category either. With a bottom of the barrel grade of F in nursing, this is about as terrible as it gets. Nursing scores are based largely on quality and quantity of nursing care. This nursing home offered only 0.3 hours of nursing care per patient each day. This is an alarmingly low total. To pair with its below average totals in the area of nursing hours per patient, this facility had really poor scores in some of the quality-based measures we looked at in calculating our nursing scores. We looked at the percent of residents having falls leading to serious injury. This nursing home was at more than 150 percent of the national average in this statistic.
