Registered Staff Hours
Nurse Quality
Although we gave this facility a terrible overall grade, we awarded it a grade of B- for our nursing rating. Our nursing grade weighs several factors, most of which are associated with quantities of nurse staffing. This facility provided just 0.3 hours of nursing care per resident per day. This is a relatively low total compared to most other facilities. Despite having low quantities of nursing care, this place actually excelled in some of the quality-based metrics we looked at. It performed well in terms of minimizing its residents' pressure ulcers and major falls. Many consider these areas to be good indicators of the quality of nursing care. In many cases, the quality of nursing care is as important as the quantity of hours of care provided.
Short-term Care Quality
We were also stunned to learn that this nursing home earned a somewhat respectable short-term care grade in our assessment. We awarded it a B- in that category. Short-term care grades are often employed to grade a facility's rehabilitation services. In order to have highly graded rehabilitation services, facilities generally must offer better levels of skilled nursing. Skilled nursing includes registered nurses, physical therapists, speech therapists and other licensed professionals. In this facility's case, we were surprised to learn that it actually provides less registered nurse and physical therapist hours per patient than the typical facility. Lastly, we assessed the number of patients who were able to return to the community from this nursing home. We found that 0 percent of this facility's patients returned home. At most nursing homes, closer to half of their residents are able to return home so this figure is below average.

Facility Inspections
We also wanted to point out this place's inspection score where it received an F. For facilities with bad inspection ratings, you should focus on any severe deficiencies found on their inspection reports. This particular nursing home was hit with 2 deficiencies by government inspectors. The only positive thing we can say is that none of these deficiencies were in the categories that indicate they created a risk to resident safety or health. Finally, CMS cited this nursing home for possible abuse or neglect. Hopefully, its performance improves in the future, but until then we simply can't recommend this facility.
Long-term Care Quality
The last category we scored was long-term care, in which this nursing home was given an abysmal grade in this area also. We gave it an F in this category, which is obviously very concerning. When nursing homes receive this kind of score in this category it is typically a bad sign for patient care and it may mean that the nursing home is not as well-staffed. One of the factors we considered on top of nursing hours was vaccines. Candidly, we were a bit concerned this facility administered the pneumonia vaccination to a relatively low 64.011795 percent of its residents. Surprisingly, this facility was actually able to limit hospitalizations. With just 0 hospitalizations per one thousand long-term resident days, this nursing home has less hospitalizations than the average nursing home. This is its best feature in this category.

Better nursing protocols can reduce the percentage of residents in a nursing home who sustain pressure ulcers.
Minimizes Serious Falls
In Pueblo Center, 4.42% of Patients had Serious Falls
This figure measures the percentage of long-term residents who sustained a fall resulting in severe injury. Falls can happen for a variety of reasons, but high rates of falls resulting in injuries could be an indicator of lower quality nursing care.

This indicates the percent of long-term residents who are given antipsychotic medication. Increased usage of these drugs may mean that a facility is using these medications to control patient behavior in situations where such medications are not medically indicated. This is the percent of patients prescribed antianxiety drugs. Antianxiety drugs are prescribed to patients experiencing anxiety or depression.
