Facility Inspections

Grade: B-plus

Although we were not high on this nursing home's overall performance, it did receive an impressive inspection report this year with few dings from the applicable government inspectors. As a result, it received one of our better grades in that area with a B+. These inspection scores weigh several factors, including deficiencies, substantiated complaints and federal fines. While this nursing home had some deficiencies on its report, none were major deficiencies based on CMS' scale. A few minor deficiencies aren't necessarily the end of the world.

Short-term Care Quality

Grade: C

This nursing home's short-term care grade is one of its best features. The facility was given a C in this category. In the area of short-term care, we seek to assess indicators of a facility's rehabilitation. We assess a facility's skilled nursing services, such as the ones performed by registered nurses and various therapists. We found that this nursing home was below average in the two key staffing areas we focus on. The facility provided fewer registered nurse and physical therapist hours per resident than most other nursing homes. Lastly, we considered the number of patients who were able to return to the community from this facility. We found that 5.1 percent of this facility's patients returned home. At most nursing homes, closer to half of their residents are able to return home so this figure is below average.

Long-term Care Quality

Grade: D

We gave this facility a grade of just D for our long-term care grade. This is not one of our better scores. If you are looking for services other than short-term rehabilitation, you should take a close look at each nursing home's long-term care grades. After assessing the quantity of care provided by nurses and other staff, we turned to the facility's vaccination record. We were pleased to discover that this nursing home vaccinated 98.36957 percent of its residents for pneumonia. To our surprise, this facility was able to limit hospitalizations. With just 0.01 hospitalizations per 1,000 long-term resident days, this nursing home had fewer hospitalizations than the average nursing home. Sadly, some of its other scores in this category were not as favorable as these.

Nurse Quality

Grade: D

The next area we looked at was nursing, which is this facility's least impressive area. We gave this nursing home a D in that area. The nursing score features several factors. The most important factor is the amount of time nurses spent with residents. This facility averaged a meager 0.3 hours of nursing care per resident on a daily basis. Unfortunately, this place also had abysmal scores in some of the quality-based measures to go along with its weak nursing hours. We looked at the percent of residents experiencing pressure ulcers and we were very disappointed. This facility was at approximately 150 percent of the national average in this measure.
