While we were not high on this facility overall, we awarded it an A for our inspections rating. This score is far more impressive than the facility's overall grade. Our inspection grades are based on pieces of information found on the a nursing home's recent inspection reports. Deficiencies are a key item to look for on these inspections. Most importantly, you should avoid nursing homes that have severe deficiencies associated with endangerment of patients. While this nursing home had some minor dings on its inspection report, it had no severe deficiencies. Severe deficiencies are ones labeled as categories G through L. This tells you that CMS did not deem any of the deficiencies on this facility's report to pose an imminent threat to patient safety or health. A couple minor deficiencies are not necessarily the end of the world.

One of this nursing home's more respectable category scores came in the area of short-term care. This proved to be its second most favorable category. In that area, we gave this facility a C. In determining these short-term care grades, we assess the nursing home's skilled nursing services, including those performed by registered nurses, occupational therapists, physical therapists and other types of skilled professionals. Our purpose is to create a barometer for sizing up the rehabilitation services of various facilities. Unfortunately, this nursing home was below average in terms of its level of registered nurse and physical therapy hours provided to its patients based on the measures we assessed. The important question is whether this adversely affected quality of care. Finally, we considered the number of patients who were able to return to the community from this nursing home. We found that 4.6 percent of this facility's residents returned home. At most nursing homes, closer to half of their residents are able to return home so this figure is below average.

Turning to the area of nursing care, this facility received a grade of just D in this category. Our nursing grade includes quite a few factors, but the paramount one is the level of nurse hours spent with patients. This place provided a meager 0.4 hours of nursing care per patient each day. This number is far below average. Finally, we also looked at some nursing quality-based metrics in computing our nursing scores. This nursing home fared well when it comes to avoiding pressure ulcers and major falls. We consider these metrics to be good measures of the quality of nursing care.

Switching gears to our final area, this facility didn't perform very well in this area either. Due to an abysmal grade of F in long-term care, this is just about as as uninspiring as it gets. For patients in need of a permanent place to live as opposed to rehabilitation, long-term care grades are an important measure. On top of assessing the volume of care provided by aids and other staff, we looked at the number of residents vaccinated for pneumonia. This facility vaccinated 100 percent of its residents, which is more than the vast majority of nursing homes. To our surprise, this nursing home also keeps its patients out of the hospital. Indeed, it had only 0.01 hospitalizations per 1,000 long-term resident days. Unfortunately, some of its other scores in this category were not as favorable as these.
