Facility Inspections
Although we gave this nursing home a terrible overall grade, we gave it a grade of B for our inspections category. This is due to the nursing home receiving a quality government inspection report this year. Arguably the most important factor we consider in determining our inspection ratings is deficiencies. These deficiencies can be found on a facility's inspection reports. It is generally best to avoid nursing homes that had a long list of deficiencies. This particular nursing home had just a single deficiency on its inspection report and it was not one deemed to pose a threat to patient health or safety. In spite of this deficiency, this place still had a relatively positive inspection report.
Short-term Care Quality
Remarkably, we gave this nursing home a grade of B- for our short-term care rating, which isn't too bad of a score. Our short-term care grades are probably most important for residents requiring rehabilitation services. Rehabilitation usually requires more highly-skilled nursing. This means not only nursing, but also physical and occupational therapy, as well as other forms of therapy. Unfortunately, we found that this nursing home provided fewer physical therapist and registered nurse hours per resident than most other facilities. The last item we considered in this category is the number of residents who were able to leave the nursing home and return to the community. We discovered that just 5.6 percent of this facility's patients were able to return home, which is below average.

Long-term Care Quality
In addition, we gave this nursing home an F in the area of long-term care. For prospective residents seeking a permanent residence rather than skilled nursing, long-term care is a very important category. After considering the volume of care provided by nurses, we turned to the nursing home's vaccination record. We were optimistic to discover that this nursing home vaccinated 99.69697 percent of its residents against pneumonia. To our surprise, this place also keeps its patients out of the hospital. It had only 0.01 hospitalizations per one thousand long-term resident days. Sadly, some of its other scores in this category weren't as favorable as these.
Nurse Quality
The next category we assessed was nursing. This facility was given an abysmal grade of F in this category. There are a variety of subcategories within this grade. Most of these subcategories are tied to staffing levels. With just 0.3 hours of nursing care per resident on a daily basis, this nursing home's nurse staffing levels were significantly below average. In addition to performing poorly in terms of nursing hours, this nursing home also had rock bottom scores in the quality-based metrics we looked at in computing our nursing ratings. For starters, we looked at the percentage of residents experiencing pressure ulcers and major falls. This facility was at more than double the national average in both of these metrics.
