To go along with its decent overall score, we awarded this facility an A+ for its inspection grade. This is far more impressive than the facility's overall rating. Our inspection grades account for a host of factors included in a facility's inspection reports. One of the most important criteria we weigh heavily is the quantity and severity of deficiencies. Places with higher grades in this category typically have few severe deficiencies. Fortunately, although this nursing home had some minor dings on its report, it had no severe deficiencies. Severe deficiencies are labeled as categories G through L. This means the inspectors did not deem any of the deficiencies on this facility's report to pose an imminent threat to patient safety or health. Finally, this facility had no substantiated complaints this year from residents, which is always a good sign.

Additionally, we gave this nursing home a grade of B+ in our long-term care category. This happens to be one of our more impressive scores. When facilities receive a score in this range in this category it generally means it has plenty of staff and is an overall good place to live on a permanent basis. On top of assessing the amount of care provided by aids and other staff, we looked at the number of residents vaccinated for pneumonia. This facility vaccinated 97.544334 percent of its residents, which is better than the average nursing home. Lastly, this nursing home was able to limit hospitalizations. Indeed, it had less than one hospitalization per one thousand long-term resident days, which is a remarkably low number.

Another unfortunate development was the F this facility was given in the area of short-term care. Our short-term care grades are based on a facility's quantity of skilled nursing services. This includes a broad range of services, spanning from registered nurses to physical and speech therapists, in addition to other variations of therapy. As you might expect, we learned that this nursing home offers significantly less physical therapist and registered nurse hours per resident than the typical facility. Lastly, we looked at the percentage of patients who were able to return to the community from this facility. This was not a strength for this nursing home. We found that just 3.3 percent of this facility's residents returned home. At most nursing homes, around half of their patients are able to return home so this is well below average.

The final category we looked at is nursing. Unfortunately, we gave it an abysmal F for this category, which is a rock bottom score. This is certainly a significant disappointment. Nursing scores are based in large part on levels of nurse staffing. This facility averaged only 0.3 hours of nursing care per resident each day. Unfortunately, this facility also had abysmal scores in several of the quality-based measures to pair with its weak nursing hour totals. We looked at the percentage of patients experiencing pressure ulcers. This nursing home was at more than double the national average in this statistic. This is concerning when you consider that many pressure ulcers are preventable with better nursing care. This ended up sealing the facility's fate of receiving a very weak nursing grade.
