Preton Trans Care at North is a small nursing home in Johnson City, Tennessee. We gave this nursing home a very impressive overall score of A+. It graded out better than many of the facilities in Johnson City by quite a bit. This is an impressive accomplishment given that the city offers some very solid facilities. We really couldn't find many negative things to say about this place. Its scores are impeccable. Short-term Care Quality
In addition to receiving a great overall score, this facility received an A+ for short-term care. In determining our short-term care ratings, we assess the nursing home's skilled nursing services, such as those performed by registered nurses, respiratory therapists, physical therapists and other types of therapists. The goal is to formulate a tool for sizing up the rehabilitation services of nursing homes. This facility excelled at the highest level in the two key staffing areas we looked at. It offered about 50% more care from physical therapists and registered nurses than the average nursing home. This is an excellent sign. Finally, we looked at the percentage of patients that returned home from this nursing home. In addition to excelling in the area of physical therapy, we found that it performed as well as just about any nursing home in the nation with 68.6 percent of its residents returning home. Most facilities are below 50 percent in this metric.
Facility Inspections
Adding to an already impressive profile, this facility also received virtually flawless health inspections in recent years. As a result, we awarded it an A+ inspection grade. We weigh several aspects of a nursing home's inspection report in calculating our inspection ratings. One of those factors is deficiencies. You should keep in mind that the severity of these deficiencies is usually more important than the number of deficiencies, as some deficiencies end up being relatively insignificant. While this nursing home had some deficiencies on its government inspection report, none were major deficiencies based on CMS' scale. Finally, this facility had no substantiated complaints this year from residents, which is an excellent sign.
Long-term Care Quality
This facility also performed well in the area of long-term care, where we gave it a score of A. Few nursing homes performed better in this area. In a long-term care environment, the nursing home's primary goal is to keep patients healthy and safe. This is different than skilled nursing or short-term care where the goal is to rehabilitate residents. One of the factors we considered after nurse's aid hours was vaccines. Unfortunately, it doesn't appear that this nursing home submitted vaccination data. Clearly, this facility is doing something right in this area as it also excelled at keeping its patients out of the hospital. In fact, it had less than one hospitalization per 1,000 long-term resident days, which is a remarkably low number.
Nurse Quality
Moving on to the fourth area, this nursing home received a highly favorable nursing grade. In fact, we awarded it an A in this area, rounding out a straight A profile. We scrutinized the qualifications of nurses employed by the nursing home, in addition to the quantity of hours those nurses spent with residents, in determining our rating in this category. This place boasts a really impressive 5.2 hours of nursing care per patient each day, of which a significant portion was provided by registered nurses. This is one of the most highly highly trained levels of nurses. On top of providing impressive levels of nursing care, this facility also excelled in the quality measures we looked at. It performed as well as any nursing home in the nation when it comes to minimizing its residents' pressure ulcers and major falls.
