Presence Heritage Village is one of only three available facilities in Kankakee, Illinois. We awarded this facility an A- overall grade, ranking it in the top quarter of all facilities in the United States. We could not find many bad things to say about this facility. Short-term Care Quality
To go along with its strong overall grade, we awarded this nursing home a grade of A for our short-term care rating. Our short-term care ratings are presumably most meaningful for individuals needing a nursing home for rehabilitation. Rehabilitation usually requires more highly-skilled nursing. Skilled nursing means not merely nursing services, but also physical and occupational therapy, as well as other forms of therapy. This facility excelled at the highest level in two key staffing areas we looked at. In fact, it provided about 50% more care from registered nurses and physical therapists than a typical facility. Lastly, we looked at the percentage of residents that ultimately were able to return home from this facility. It performed better than most facilities in the nation in this area with 53.7 percent of its residents able to return home.
Facility Inspections
Turning to inspections, this nursing home excelled in that area, which is based on the nursing home's recent inspections. We awarded it an A for that category. We take several aspects of a facility's inspection report into consideration in calculating our inspection grades. One key factor is deficiencies. You should keep in mind that the severity of deficiencies is usually more important than the quantity of deficiencies, as some deficiencies end up being quite minor. This nursing home was hit with 5 deficiencies on its inspection report, but we were relieved to see that none of the deficiencies were considered to be major deficiencies. This tells you that the inspectors did not consider any of these deficiencies to create an imminent risk to resident safety or health. The fact that none of the deficiencies were severe made us feel better about this inspection report.
Nurse Quality
Adding to its impressive resume, this nursing home also excelled in our nursing category. In fact, we awarded it a grade of A- for that category. Our nursing score includes several components, however, the paramount consideration is the number of nurse hours per patient per week. This facility provides an incredible 4.4 hours of nursing care per resident daily. Very few nursing homes provide this quantity of nursing care to their patients. Out of that total, many of the hours were provided by registered nurses, which is one of the most highly highly trained levels of nurses. On top of providing impressive levels of nursing care, this facility also excelled in several of the quality-based metrics we looked at. With less than five percent of its residents suffering from pressure ulcers, it performed as well as any place the state in this category. This is usually a good indicator that a place has reliable quality controls in place. Many pressure ulcers could be avoided by providing better nursing care and having a policy of moving residents even once per day.
Long-term Care Quality
The last category we rated is long-term care. We gave this nursing home a decent score in that area, with a grade of B-. Even though this was not as high as several of this nursing home's other scores, it is nevertheless nothing to be ashamed of. In a long-term care environment, the nursing home's primary objective is to maintain residents' quality of life and keep them safe. One of the factors we considered after this facility's very solid nursing hours was vaccines. This nursing home administered the pneumonia vaccination to 100 percent of its residents. Vaccination is a great way to minimize unnecessary deaths for the senior population. Lastly, we looked at the facility's number of hospitalizations. We found that this facility had 2.98 hospitalizations per 1,000 long-term resident days. Although this figure is somewhat concerning, this number can be skewed for some facilities based on the preexisting medical conditions of residents.
