While this place's overall score was terrible, it fared well in the category of inspections. In fact, we awarded it a grade of B+ for that area, which is one of our better scores. Our inspection scores are based on several datapoints located in the a nursing home's recent inspection reports. Deficiencies are a key thing to look for on these reports. You should especially avoid nursing homes with severe deficiencies linked to endangerment of patients. Fortunately, although this nursing home had a few minor dings on its inspection report, it had no severe deficiencies. Severe deficiencies are labeled as categories G, H, I, J, K and L. This means CMS didn't deem any of the deficiencies on this facility's report to pose an imminent threat to patient health or safety. A couple minor deficiencies aren't necessarily the end of the world.

This facility's second best score is short-term care, but even there it received just a D in that area. This ended up being a well below average grade. In crafting our short-term care grades, we assess the facility's skilled nursing services, such as those performed by registered nurses, speech therapists, physical therapists and other types of therapists. The purpose is to create a scale for comparing the rehabilitation services of various nursing homes. In this nursing home's case, we found that it provides less physical therapist and registered nurse hours per resident than the average facility. Lastly, we considered the number of residents who ultimately returned home from this nursing home. This nursing home did not fare well here either. We found that just 48.4 percent of this facility's residents were able to return home. This figure was below the national average.

The third category we graded was long-term care. This nursing home's received a low-end long-term care grade where it was given an F. If you are looking for services other than short-term rehabilitation, you should take a close look at each facility's long-term care ratings. Once we assessed the volume of care provided by nurses and other staff, we turned to the nursing home's vaccination data. We were pleased to learn that this facility vaccinated 100 percent of its residents for pneumonia. To our surprise, this nursing home was able to limit hospitalizations. With only 1.31 hospitalizations per one thousand long-term resident days, this facility has less hospitalizations than most nursing homes. Sadly, some of its other scores in the category weren't as favorable as these.

The final category we rated was nursing. This nursing home was given a rock bottom grade of F in this area. Our nursing rating is primarily tied to the nursing home's nurse staffing. This facility averaged just 2.3 hours of nursing care per patient daily. In addition to receiving below average scores for nursing hours, this nursing home was less impressive in some of the quality-based measures we looked at in determining our nursing grades. We looked at the percentage of patients sustaining pressure ulcers and found this place was at approximately 1.5 times the national average in this metric. This is likely a bad sign when you consider that so many pressure ulcers are preventable with better nursing care. Unfortunately, this statistic really dragged down this nursing home's nursing rating substantially.
