Facility Inspections
While we were not complementary of this place's overall performance, it actually received an impressive inspection report this year with few dings from the applicable government inspectors. As a result, it received one of our better grades in that area with a grade of B. We weigh several aspects of a nursing home's inspection report in determining our inspection ratings. One of those factors is health deficiencies. You should keep in mind that the severity of the deficiencies is usually more important than the number of deficiencies, as some end up being quite minor. While this nursing home had some deficiencies on its inspection report, none of them were major deficiencies based on CMS' scale. A couple minor deficiencies aren't the end of the world.
Nurse Quality
We also gave this facility favorable marks in the area of nursing care, where it received a grade of B. Our nursing score is mostly associated with the facility's nurse staffing. This facility provided 0.3 hours of nursing care per resident per day. Finally, our nursing grades also factor in quality-based assessments, such as avoiding major falls. This facility performed well in this area. Avoiding major falls is typically a good indicator that a facility has reliable quality controls in place. Major falls can often be avoided if more nurses aids and better safety protocols are in place.

Long-term Care Quality
Unfortunately, this facility only earned a grade of D for its long-term care rating, which is not an impressive grade. When facilities receive a grade in this range in this category it generally means the facility didn't perform well in our quality measures relating to resident care. On top of looking at the volume of care provided by aids and other staff, we also looked at the percentage of residents vaccinated for pneumonia. We discovered that this nursing home vaccinated just 82.30088 percent of its residents, which is several points lower than we expected. To our surprise, this nursing home actually fared well at keeping its patients out of the hospital. In fact, it had just 0.01 hospitalizations per 1,000 long-term resident days. This was its best feature in this category.
Short-term Care Quality
Moving on to the fourth area, we gave this facility a D for the area of short-term care. This is the facility's worst category rating. Our short-term care grade is generally used to score a nursing home's rehabilitation services. To offer high-end rehabilitation services, nursing homes generally must feature better levels of highly skilled nursing services. These types of services include registered nurses, physical therapists, occupational therapists and other licensed professionals. Not surprisingly, we found that this nursing home provides far less physical therapist and registered nurse hours per resident than the average facility. Finally, we considered the percentage of patients who who were able to eventually return home from this nursing home. This nursing home didn't fare well here either. In fact, we found that just 3.8 percent of this facility's patients were able to return home. This figure was quite a bit off the national average.
