This is comparable with the city grade in Toledo, which is a B-. You could certainly do worse than this place. Finally, this facility had an ownership change in the last year, so we'll have to see if that has any effect on its grades in the coming year.

Facility Inspections
We want to point out that this nursing home really outperformed its overall score in the area of inspections. In fact, it received a nearly flawless inspection report this year. As a result, it received one of our highest grades in this category with an A. Inspection scores are tied to several items found on the a nursing home's recent inspection reports. Facilities that receive favorable grades in this category tend to have very few deficiencies on their reports. Most importantly, these nursing homes should not have any severe deficiencies which are associated with endangerment of patients. This facility received 3 deficiencies on its inspection report, but we were relieved to see that none of the deficiencies were considered to be severe deficiencies. This indicates that CMS didn't deem any of these deficiencies an imminent risk to resident safety or health. A few minor deficiencies shouldn't lead you to cross a nursing home off your list.
Short-term Care Quality
This facility was given its second most favorable category grade in the area of short-term care. We gave this facility a B- in this area, which is a reasonably acceptable score. Short-term care ratings are crucial for patients looking for rehabilitation. Rehabilitation typically mandates more skilled nursing services. This means a wide scope of nursing services, spanning from registered nurses to physical and speech therapists, as well as other variations of therapy. We were pleased to find that this nursing home employs both registered nurses and physical therapists. The same can't be said for all nursing homes. Finally, we looked at the percentage of residents who were able to return to the community from this facility. This proved to be more of a strength for this facility. In fact, we found that it outperformed the majority of nursing homes in the nation with 49.4 percent of its residents returning home.
Long-term Care Quality
As it turns out, this facility only earned a grade of D for its long-term care rating, which is not one of our better scores. Nursing homes that do not fare well in long-term care often don't provide as much nursing care and also may be lagging in some of the areas of routine medical care we looked at. In addition to considering the amount of care provided by nurses aids and other staff, we analyzed the number of residents vaccinated against pneumonia. This nursing home provided the vaccine to 92.68293 percent of its patients. This is a bit lower than we were hoping for but still a reasonably acceptable percentage. Surprisingly, this facility was actually able to limit hospitalizations. With just 0 hospitalizations per one thousand long-term resident days, this facility has less hospitalizations than the average nursing home. This is its best feature in this category.
Nurse Quality
The last category we looked at is nursing, which proved to be a weak spot for this nursing home. We awarded this nursing home a grade of just D for our nursing category. Our nursing grade is based on the facility's nurse staffing levels. We factor in both the levels of licensure of the nurses as well as the amount of time spent with patients. This facility averaged just 3 hours of nursing care per patient daily. To go along with its low totals in the area of nursing hours per patient, this nursing home also didn't fare as well in the quality-based measures we looked at in determining our nursing grades. We looked at the percentage of patients sustaining major falls and pressure ulcers. This nursing home had more falls and pressure ulcers than the average nursing home.
