Facility Inspections

Grade: C

Although we gave this facility a terrible overall grade, we awarded it a grade of C for our inspections rating. In fact, the facility received very impressive inspection report this year. Our inspection scores account for several factors found on a facility's inspection report. One of the most important criteria we look at is the quantity and severity of deficiencies. This facility was assessed 4 deficiencies on its government inspection report, but we were relieved to find that none were in any of the categories labeled G through L. This indicates that CMS didn't deem any of the deficiencies to present an imminent risk to resident safety or health. Unfortunately, we also need to draw your attention to the fact that this facility received significant government fines in recent years.

Nurse Quality

Grade: D

Turning to the area of nursing care, this facility received a grade of just D in this category. Unfortunately, this ended up being one of its better grades. Our nursing score is mostly based on the nursing home's nurse staffing. This facility offered just 0.3 hours of nursing care per patient each day. This number is far below average. Lastly, we also assessed some nursing quality-based metrics in computing our nursing scores. This facility fared well in the area of minimizing its residents' major falls and pressure ulcers. We consider these metrics to be reliable indicators of the quality of nursing care.

Long-term Care Quality

Grade: F

Our next category we analyzed is long-term care, where this facility was given an F. For long-term care residents, the nursing home's primary goal is to maintain residents' quality of life and keep them safe. Once we concluded our assessment of the volume of nursing care, we then looked at the nursing home's vaccination data. We were optimistic to discover that this facility vaccinated 94.89051 percent of its patients for pneumonia. To our surprise, this facility was able to keep its patients out of the hospital. In fact, it had just 0.03 hospitalizations per one thousand long-term resident days. Sadly, some of its other scores in this category weren't as favorable as these.

Short-term Care Quality

Grade: F

The final area we rated is short-term care. It received an F in this category, which is an abysmal score. In the category of short-term care, we strive to assess indicators of a facility's rehabilitation services. We look at a facility's skilled nursing services, such as the ones performed by registered nurses and physical, occupational and other therapists. Given its poor score in this category, we were not shocked to find this place provided substantially fewer physical therapist and registered nurse hours per resident than most other facilities. Lastly, we assessed the percentage of residents who were able to return to the community from this nursing home. We found that just 3.6 percent of this facility's residents returned home.
