Facility Inspections

Grade: B-minus

While this place's overall grade was as bad as it gets, it actually didn't perform terribly in the area of inspections. In fact, we awarded it a B- for that category, which is one of our better scores. Inspection ratings account for several factors included in a nursing home's inspection reports. One of the most important criteria we weigh heavily is the quantity and severity of deficiencies. Fortunately, although this nursing home had some minor dings on its government inspection report, it had zero severe deficiencies. Severe deficiencies are those labeled as categories G, H, I, J, K and L. This tells you the inspectors did not consider any of the deficiencies on this facility's report to pose an imminent threat to patient health or safety. Lastly, this nursing home was cited by CMS for possible abuse or neglect. This is generally a really bad sign. We would never recommend this facility.

Short-term Care Quality

Grade: D

This facility's second most respectable category is short-term care, but it nevertheless received just a D in that category. This turns out to be a well below average score. With our short-term care score, we try to forge a sound barometer for rehabilitation. In this process, we analyze the facility's offerings of skilled nursing services, including both registered nurses and physical therapy, as well as occupational therapy. Based on its grade in this category, we weren't stunned to learn that this nursing home is well below average in terms of the quantity of physical therapy and registered nurse hours provided to its residents based on the data we assessed. Finally, we considered the percentage of patients who were able to return to the community from this facility. This wasn't a strength for this nursing home. We found that just 4.1 percent of this facility's residents returned home. At most facilities, about half of their residents are able to return home so this is well below average.

Nurse Quality

Grade: F

This facility ended up receiving rock bottom score in our nursing category. It received an F in this category. When calculating our nursing grades, we factor in both the number of hours nurses spend with residents and the levels of licensure of the nurses. This particular place averaged just 0.3 hours of nursing care per patient per day. This statistic is quite a bit below average. Lastly, we also looked at some nursing quality measures in calculating our nursing scores. This facility performed well in the area of minimizing its residents' major falls and pressure ulcers. We consider these metrics to be reliable measures of the quality of nursing care.

Long-term Care Quality

Grade: F

The next area we looked at was long-term care. This nursing home was given a poor grade of F in this category. In a long-term care setting, the primary objective is to keep patients as healthy and safe as possible. This is different than skilled nursing or short-term care where the goal is to rehabilitate patients. After we finished looking at the amount of care provided by nurses, we then looked at the nursing home's vaccination record. We were pleased to find that this nursing home vaccinated 96 percent of its patients for pneumonia. To our surprise, this facility also keeps its residents out of the hospital. In fact, it had only 0.01 hospitalizations per 1,000 long-term resident days. Unfortunately, some of its other scores in the category were not as favorable as these.
