Long-term Care Quality

Grade: A-plus

On top of being a first rate facility overall, this facility also excelled in long-term care, where it earned a grade of A+. Few nursing homes fared better in this category. When facilities receive a grade in this range in this category it typically means it's well-staffed and is a quality place to live on a permanent basis. One of the statistics we considered in addition to nursing hours was vaccinations. This facility provided the pneumonia vaccine to 98.25784 percent of its patients. Vaccination is a proven method to avoid unnecessary deaths and hospitalizations for the nursing home population. Lastly, this nursing home was able to limit hospitalizations. Indeed, it had less than one hospitalization per 1,000 long-term resident days, which is a remarkably low number.

Short-term Care Quality

Grade: A-plus

We also awarded this facility a grade of A+ for its short-term care grade. Our short-term care grade is commonly employed to assess a nursing home's performance with rehabilitation. To provide highly rated rehabilitation services, nursing homes generally must have higher levels of skilled nursing. These types of services include registered nurses, physical therapists, respiratory therapists and other licensed professionals. Fortunately, it looks like this nursing home has registered nurses on staff. Not all nursing homes employs these skilled professionals. However, based on the information this nursing home provided, it does not look like the facility employs physical therapists. The last metric we looked at in this category is the number of patients that eventually returned home from the nursing home. We found that 6.2 percent of this nursing home's patients returned home as opposed to remaining at the nursing home on a permanent basis.

We also wanted to emphasize the fact that this facility excelled in our nursing rating. We awarded them an impressive grade of A+ in this category. We weighed the skill-level of nurses employed by the facility, in addition to the quantity of hours the nurses were with residents, in calculating our grade in this area. This particular nursing home provided 0.5 hours of nursing care per resident per day. Finally, our nursing grades also factor in quality-based assessments, such as avoiding major falls. This nursing home performed well in this area. Avoiding major falls is typically a good indicator that a facility has reliable quality controls in place. Major falls can generally be avoided if more nurses aids and better safety protocols are in place.

Facility Inspections

Finally, this facility also earned a virtually flawless government inspection. As a result, it received one of our highest grades in that category with an A. With this impressive grade, this place is one of the few facilities in Minnesota to earn straight A's. We take several aspects of a facility's inspection report into consideration in calculating our inspection grades. One of those factors is deficiencies. You should keep in mind that the severity of deficiencies is usually more important than the number of deficiencies, as some end up being relatively minor. This nursing home received 8 deficiencies on its inspection report, but none of the deficiencies were considered to be severe. This tells you that CMS did not deem any of the deficiencies to cause an immediate threat to resident health or safety. Finally, this facility had no substantiated complaints this year from residents, which is an excellent sign.
