In addition to receiving a bad overall score, we also awarded this facility terrible grades in the various categories. In fact, it only received a D for its inspections score, which was its top area. One of the most important factors in assessing an inspection report is deficiencies. We would scrutinize the level of severity of those deficiencies. This particular facility was assessed 6 deficiencies by government inspectors. The only positive thing we can say is that none of these deficiencies were in the categories that suggest that they posed a threat to resident safety or health. Finally, this facility also received 7 substantiated complaints in recent years. This is yet another bad sign.

Among this facility's litany of poor grades was its F in the area of short-term care. With our short-term care assessment, we seek to craft a fair gauge for rehabilitation services. In this process, we analyze the nursing home's scope of skilled nursing services, including both registered nurses and physical therapy, as well as respiratory therapy. As you might expect, we learned that this nursing home provides significantly less physical therapist and registered nurse hours per resident than the typical nursing home. Finally, we assessed the percentage of patients who were able to return to the community from this facility. We found that just 38.9 percent of this nursing home's patients returned home.

In addition, we gave this facility a rock-bottom F in the area of long-term care. In a long-term care environment, the facility's primary objective is to maintain patients' quality of life and keep them safe. After assessing the volume of care provided by aids and other staff, we then analyzed the facility's vaccination records. We were optimistic to find that this nursing home vaccinated 100 percent of its residents for pneumonia. To our surprise, this nursing home was able to limit hospitalizations. With just 1.08 hospitalizations per one thousand long-term resident days, this nursing home had fewer hospitalizations than many nursing homes. Sadly, a few of its other scores in this category weren't as favorable as these.

The final area we rated is nursing. This facility received a poor grade of F in this category. When determining a nursing home's nursing score, we weigh the number of hours nurses commit to residents as well as the skill levels of the nurses. This nursing home provides 3.1 hours of nursing care per resident per day. This is a much lower figure than we are used to seeing. To go along with its low totals in the area of nursing hours per patient, this facility also didn't fare as well in the quality-based measures we looked at in computing our nursing grades. We looked at the percentage of residents experiencing pressure ulcers and major falls. This facility had more falls and pressure ulcers than the average nursing home. This may be a bad sign when you consider that many bed sores and falls are believed to be preventable with better nursing care. Unfortunately, these scores hurt this facility's nursing grade quite a bit.
